二三止哮汤对支气管哮喘慢性持续期(痰浊阻肺证)的改善效果及对Th1/Th2平衡及相关细胞因子的影响

摘　　要：目的:探究二三止哮汤对支气管哮喘慢性持续期(痰浊阻肺证)的改善效果及对Th1/Th2平衡及相关细胞因子的影响。方法:选择2020年6月~2021年6月我院收治的的110例哮喘患者,按照随机数字表法分为联合组(沙美特罗替卡松粉吸入剂+二三止哮汤)和西医组(沙美特罗替卡松粉吸入剂),各55例,比较两组临床疗效、中医证候积分、Th1/Th2及其相关细胞因子水平和不良反应发生情况。结果:联合组临床疗效等级比较显著优于西医组(P<0.05),总有效率高于西医组(P<0.05)。两组主症、次症、体征等中医证候积分均较治疗前下降(P<0.05),且联合组显著低于西医组(P<0.05)。两组Th1、Th1/Th2、血清γ干扰素(IFN-γ)、白细胞介素-2(IL-2)均较治疗前上升(P<0.05),联合组显著高于西医组(P<0.05),Th2、白细胞介素-4(IL-4)、白细胞介素-10(IL-10)均较治疗前下降(P<0.05),联合组显著低于西医组(P<0.05)。两组患者不良反应发生情况无统计学意义(P>0.05)。结论:二三止哮汤可以通过调节Th1/Th2平衡,促进IFN-γ、IL-2,抑制IL-4、IL-10来提高支气管哮喘慢性持续期(痰浊阻肺证)的临床疗效。Objective:To explore the improvement effect of Ersan Zhixiao Decoction for chronic persistent bronchial asthma(phlegm turbidity obstructing lung syndrome),and its influence on Th1/Th2 balance and related cytokines.Methods:110 patients with asthma admitted to the hospital between June 2020 and June 2021 were selected,and divided into a combined group(Salmeterol and Fluticasone Powder for inhalation combined with Ersan Zhixiao Decoction)and a western medicine group(Salmeterol and Fluticasone Powder for inhalation)by random number table method,with 55 cases in each group.Clinical effect,TCM syndrome scores,Th1/Th2,related cytokines and adverse reactions were compared between the two groups.Results:The total response rate of the combined group was higher than that of the western medicine group(P<0.05).The scores of main symptoms,secondary symptoms,and physical signs of the two groups were reduced after treatment(P<0.05),and the combined group had significantly lower scores than the western medicine group(P<0.05).After treatment,Th1,Th1/Th2,serum interferon gamma(IFN-γ),and interleukin-2(IL-2)in the two groups were increased(P<0.05),which were significantly higher in the combined group than in the western medicine group(P<0.05).Th2,interleukin-4(IL-4),and interleukin-10(IL-10)were reduced(P<0.05),and were significantly lower in the combined group than in the western medicine group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Ersan Zhixiao Decoction can improve the clinical effect on chronic persistent bronchial asthma(phlegm turbidity obstructing lung syndrome)by regulating Th1/Th2 balance,promoting IFN-γand IL-2,and inhibiting IL-4 and IL-10.
