从“玄府主气液宣通”论治干燥综合征

摘　　要：干燥综合征归属于中医燥痹范畴,病程长,迁延难愈,严重影响患者身心健康及生活质量。大多医家认为阴虚燥热为主要病机,以滋阴润燥为基本治法对部分患者疗效欠佳。玄府关乎全身气液通利,贵开忌阖,玄府闭塞气液阻滞则百病皆生。笔者认为玄府闭塞,气液宣通受阻是干燥综合征的病机关键。内外燥邪相合煎灼津液,玄府郁闭是始动环节;气机升降失常,玄府闭塞是病程转折;燥毒瘀互结,壅滞玄府贯穿疾病始终,是发生变证坏证的主要因素。治疗上主要有内外治通玄,内治以生津润燥养玄、活血解毒通玄、温通阳气启玄为法,外治以辛味药物为主结合熏洗溻渍为法。运用玄府气液理论指导干燥综合征的治疗,以期为中医药治疗干燥综合征提供新的思路。Sjogren’s syndrome belongs to the category of dryness and arthralgia in traditional Chinese medicine.It has a long course of disease and is difficult to cure,which seriously affects the physical and mental health and quality of life of patients.Most doctors think that Yin deficiency and dryness heat is the main pathogenesis,and the basic treatment of nourishing Yin and moistening dryness has poor effect on some patients.Xuanfu is related to the circulation of Qi and liquid in the whole body.The author thinks that the key pathogenesis of Sjogren’s syndrome is the obstruction of Xuanfu and the obstruction of Qi-liquid circulation.The combination of internal and external dryness and pathogenic factors is the starting link of body fluid and Xuanfu stagnation.Abnormal Qi movement and Xuanfu block are the turning point of the course of disease.The combination of dryness,toxin and blood stasis and the stagnation of Xuanfu run through the whole disease,which are the main factors leading to the change and deterioration of syndrome.The treatment mainly includes internal and external treatment for Xuantong,internal treatment for Shengjin Runzao Yangxuan,Huoxue Jiedu Tongxuan,Wentong Yangqi Qixuan,external treatment for pungent drugs combined with fumigation and pickling.Xuanfu Qi-liquid theory was used to guide the treatment of Sjogren’s syndrome,in order to provide a new idea for TCM treatment of Sjogren’s syndrome.
