曹吉勋教授采用“消、托、补”分期论治放射性直肠炎经验浅析

摘　　要：“消、托、补”为中医外科常用三大治疗法则,大多数医生将其应用于体表疮疡的外治之中。曹吉勋教授从事中医肛肠专业六十余年,认为“消、托、补”理论不仅是体表疮疡的外治法则,还可以将其广泛运用于胃肠脏腑疾病的内治之中。随着放射性治疗在盆腔恶性肿瘤患者中广泛应用,由此带来的放射性直肠炎的发病率也逐渐提高。曹吉勋教授认为该病本虚标实,证候虚实转化。初期“火毒侵袭、肠络受损”,治以“消”为先导;中期“正邪交争、虚实夹杂”,治以“托”为转折;后期“久病耗伤、脾肾两虚”,治以“补”为根基。本文从消、托、补三法溯源与新释、精准分期论治两方面浅析曹吉勋教授于放射性直肠炎中对消、托、补三法的涵义扩充认识和精准应用。“Eliminating,supporting and supplementing”are the three commonly used surgical treatment principles of Traditional Chinese medicine,and most doctors apply them to the external treatment of body surface ulcers.Professor CAO Jixun,who has been specializing in anorectal medicine for more than 60 years,believes that the theory of“eliminating,supporting and supplementing”is not only the external treatment rule of body surface ulcer,but also can be widely used in the internal treatment of gastrointestinal diseases.With the extensive application of radiation therapy in the patients with pelvic malignant tumor,the incidence of radiation proctitis brought by this disease is also increasing gradually.Professor CAO Jixun believes that this disease is characterized by deficiency and syndrome transformation.In the early stage of“fire poison invasion,intestinal collateral damage”,treatment with“Xiao”as the forerunner.In the middle period,“the conflict between good and evil,the mixture of false and reality”,and“Trust”as the turning point of governance.Late“long illness consumption injury,spleen and kidney deficiency”,treatment to“Fill”as the foundation.This paper analyzes professor CAO Jixun’s understanding and accurate application of the three methods of elimination,support and supplement in radiation proctitis from two aspects:tracing the origin of elimination,support and supplement,new interpretation and accurate staging treatment.
