自拟疏肝通窍方联合甲泼尼龙琥珀酸钠耳后注射治疗突发性耳聋(肝火上扰型)的疗效及预后观察

摘　　要：目的:研究自拟疏肝通窍方联合耳后注射甲泼尼龙琥珀酸钠治疗突发性耳聋(肝火上扰型)的疗效和预后。方法:选取2018年1月~2021年1月在我院确诊为突发性耳聋的120例患者作为研究对象,运用随机数字表法分为观察组和对照组各60例,对照组采用甲泼尼龙琥珀酸钠耳后注射治疗方法,观察组在对照组的基础上采用自拟疏肝通窍方联合治疗,比较两组患者的治疗效果和预后。结果:两组患者临床治疗总有效率为98.33%和88.33%,观察组疗效明显优于对照组(P<0.05);治疗后,两组患者中医证候积分均降低,且观察组降低幅度大于对照组(P<0.05);治疗后,两组患者纯音听阈值均有下降,且观察组下降幅度明显大于对照组(P<0.05),观察组患者听力改善时间短于对照组(P<0.05);观察组和对照组不良反应发生率为3.33%和15.00%,观察组明显小于对照组(P<0.05)。结论:自拟疏肝通窍方联合甲泼尼龙琥珀酸钠耳后注射治疗突发性耳聋(肝火上扰型)疗效显著,听力情况恢复良好,有利于患者预后。Objective:To study the efficacy and prognosis of using Self-made Shugan Tongqiao Decoction combined with Postauricular Injection of Methylprednisolone Sodium Succinate in the treatment of liver fire disturbance type sudden deafness.Methods:120 patients who were diagnosed with sudden deafness in the hospital between January 2018 and January 2021 were selected as the research subjects.According to the random number table method,the patients were divided into an observation group and a controlled group with 60 cases in each group.The controlled group was given Postauricular Injection of Methylprednisolone Sodium Succinate,and the observation group was combined with Self-made Shugan Tongqiao Decoction on the basis of the controlled group.The treatment effect and prognosis of the two groups of patients were compared.Results:The total clinical treatment effective rates of the two groups of patients were 98.33%and 88.33%,and the efficacy of the observation group was significantly better than that of the controlled group(P<0.05).After treatment,the scores of TCM syndromes were reduced in the two groups,and the reductions in the observation group were greater than those in controlled group(P<0.05).After treatment,the pure tone hearing threshold of the two groups of patients was decreased,and the decrease in the observation group was significantly greater than that in control group(P<0.05),and the hearing improvement time was shorter in observation group than that in the controlled group(P<0.05).The incidence rate of adverse reactions of 3.33%in the observation group was significantly lower than 15.00%in the controlled group(P<0.05).Conclusion:Self-made Shugan Tongqiao Decoction combined with Postauricular Injection of Methylprednisolone Sodium Succinate has a significant efficacy and good recovery of hearing condition in treating liver fire disturbance type sudden deafness,and it is beneficial to the prognosis of patients.
