甘麦大枣汤合二至丸加减联合雌孕激素治疗更年期综合征的效果及对睡眠质量的影响

摘　　要：目的:探究甘麦大枣汤合二至丸加减联合雌孕激素治疗更年期综合征的效果及对睡眠质量的影响。方法:选择2019年2月~2021年2月于我院就诊的更年期综合征患者114例,采用抽签法分为联合组与常规组各57例,常规组患者给予雌孕激素治疗,联合组在雌孕激素治疗的基础上给予甘麦大枣汤合二至丸加减治疗。治疗前、治疗1个月及治疗3个月后,比较两组患者的女性绝经期自测表(Kupperman改良)评分;治疗前后,比较两组患者的中医证候积分、匹兹堡睡眠质量指数(PSQI)评分、促卵泡激素(FSH)、促黄体激素(LH)及雌二醇(E_(2))水平。结果:治疗前,两组患者Kupperman评分对比,差异无统计学意义(P>0.05),治疗后1个月及3个月,联合组Kupperman评分低于常规组(P<0.05);治疗后,两组患者潮热盗汗、腰膝酸软、烦躁易怒、失眠多梦等积分均下降,且联合组患者上述各项积分均低于常规组(P<0.05);治疗前,两组患者FSH、LH及E_(2)水平比较,差异无统计学意义(P>0.05),治疗后,联合组患者FSH、LH水平均小于常规组,E_(2)水平大于常规组(P<0.05);治疗前,两组患者PSQI各项及总分比较,差异无统计学意义(P>0.05),治疗后,联合组患者上述评分均低于常规组(P<0.05)。结论:甘麦大枣汤合二至丸加减联合雌孕激素治疗可降低更年期综合征患者的中医证候积分,改善更年期症状,有助于改善雌激素水平,提高睡眠质量。Objective:To explore the effect of using Modified Ganmai Dazao Decoction and Erzhi Pill combined with Estrogen and Progesterone in the treatment of menopausal syndrome and its influence on sleep quality.Methods:114 patients with menopausal syndrome who were treated in the hospital were enrolled between February 2019 and February 2021,and were divided into a combined group(957 cases)and a conventional group(57 cases)by lottery method.The patients in the conventional group were given Estrogen-progesterone therapy,and the patients in the combined group were treated with Modified Ganmai Dazao Decoction and Erzhi Pill on the basis of Estrogen-progesterone therapy.The score of female menopause self-assessment scale(Kupperman modified scale)before treatment and after 1 month of treatment and 3 months of treatment as well as scores of TCM syndromes,scores of Pittsburgh Sleep Quality Index(PSQI)and levels of follicle-stimulating hormone(FSH),luteinizing hormone(LH)and estradiol(E_(2))before and after treatment were compared between the two groups.Results:Before treatment,there was no statistical significance in Kupperman score between the two groups(P>0.05).After 1 month and 3 months of treatment,the Kupperman score in the combined group was lower than that in the conventional group(P<0.05).The scores of tidal fever and night sweating,soreness and weakness of waist and knees,anxiety and irritability and insomnia and dreaminess were decreased in the two groups after treatment,and the above scores of patients in the combined group were lower compared with those in the conventional group(P<0.05).There were no statistical differences in the levels of FSH,LH and E_(2)between the two groups before treatment(P>0.05),and the levels of FSH and LH in the combined group after treatment were lower than those in the conventional group while the level of E_(2)was higher than that in the conventional group(P<0.05).The differences in the scores of PSQI items and PSQI scale were not statistically significant before treatment between the t
