凉血清肺方结合西药外敷治疗脂溢性皮炎疗效观察

摘　　要：目的:观察凉血清肺方联合氟芬那酸丁酯软膏外用治疗脂溢性皮炎(SD)的临床疗效,并探讨其对皮肤屏障功能及血清干扰素γ(INF-γ)、白细胞介素6(IL-6)水平的影响。方法:选取2019年8月~2021年8月本院收治的86例肺胃热盛型SD患者,随机分为对照组(n=43)和观察组(n=43),对照组给予氟芬那酸丁酯软膏外用治疗,观察组在对照组基础上联合使用凉血清肺方治疗。比较两组临床疗效,临床症状评分,皮肤屏障功能[经皮水分丢失量(TEWL)、皮脂量和皮肤含水量],血清INF-γ和IL-6,皮肤病生活质量指数(DLQI)评分及不良反应。结果:观察组治疗总有效率高于对照组(90.70%vs 74.42%)(P<0.05);观察组治疗1周、2周和4周后的临床症状评分均显著低于对照组(P<0.05);治疗4周后,两组TEWL、皮脂量均明显降低,皮肤含水量增加,且观察组改善优于对照组(P<0.05);两组IFN-γ、IL-6水平均降低,且观察组低于对照组(P<0.05);观察组DLQI评分低于对照组(P<0.05);两组总不良反应发生率(11.63%vs 6.98%)比较,差异无统计学意义(P>0.05)。结论:凉血清肺方联合氟芬那酸丁酯软膏治疗肺胃热盛型SD患者疗效确切,能有效改善皮肤屏障功能和皮损症状,减少炎症细胞因子水平,提升生活质量水平。Objective:To observe the clinical curative effect of Liangxue Qingfei Decoction combined with Flufenamic Acid Butyl Ointment on seborrheic dermatitis(SD),and exploring its influences on skin barrier function,serum interferon-γ(INF-γ)and interleukin-6(IL-6).Methods:A total of 86 patients with SD of lung-stomach heat hyperthermia enrolled between August 2019 and August 2021 were randomly divided into a controlled group(n=43)and an observation group(n=43).The controlled group was treated with Flufenamic Acid Butyl Ointment,while the observation group was additionally treated with Liangxue Qingfei Decoction.The clinical curative effect,scores of clinical symptoms,skin barrier function[transepidermal water loss(TEWL),sebum content,skin water content],serum INF-γand IL-6,score of dermatology life quality index(DLQI)and adverse reactions were compared.Results:The total response rate was higher in the observation group(90.70%vs 74.42%)(P<0.05).After 1 week,2 weeks and 4 weeks of treatment,clinical symptoms scores were lower in the observation group(P<0.05).After 4 weeks of treatment,TEWL and sebum content were significantly decreased,while skin water content was increased in both groups,with a better improvement in the observation group(P<0.05).The levels of IFN-γand IL-6 in both groups were decreased,which were lower in the observation group than the controlled group(P<0.05).DLQI score was lower in the observation group(P<0.05).No significant difference was found in total adverse reaction incidence between the two groups(11.63%vs.6.98%)(P>0.05).Conclusion:The curative effect of Liangxue Qingfei Decoction combined with Flufenamic Acid Butyl Ointment is significant for patients with SD of lung-stomach heat hyperthermia,which can effectively improve skin barrier function and skin lesions,reduce levels of inflammatory cytokines and improve quality of life.
