解毒定痛方预防肺癌合并骨转移患者放疗爆发痛发生的临床效果研究

摘　　要：目的:探究解毒定痛方预防肺癌(lung cancer,LC)合并骨转移患者放疗爆发痛发生的临床效果。方法:选取2019年2月~2021年2月就诊于我院行放疗治疗的85例LC合并骨转移患者,根据随机数字表法分为对照组42例和观察组43例,对照组采用西药镇痛,观察组在其基础上采用解毒定痛方。比较两组爆发痛发生次数、疼痛数字评分(numerical rating scale,NRS);临床疗效;卡氏功能状态评分标准(Karnofsky performance status,KPS)评分;不良反应。结果:与对照组比较,观察组爆发痛发生次数和NRS评分显著减少,差异有统计学意义(P<0.05);与对照组(50.00%)比较,观察组客观缓解率(objective response rate,ORR)(62.79%)显著升高,差异有统计学意义(P<0.05);与治疗前比较,放疗后两组KPS评分显著升高,观察组升高幅度显著大于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较差异无统计学意义(P>0.05)。结论:解毒定痛方不仅能够有效预防LC合并骨转移患者放疗爆发痛,而且还能提高放疗治疗疗效、改善患者功能状态,同时不良反应少,具有较高的临床参考价值。Objective:To explore the clinical effect of Jiedu Dingtong Decoction in preventing the occurrence of radiation-induced pain in lung cancer(LC)patients with bone metastasis.Methods:From February 2019 to February 2021,85 cases of LC patients with bone metastases were randomly divided into a controlled group(42 cases)and an observation group(43 cases).The controlled group was treated with western medicine,and the observation group was treated with Jiedu Dingtong Prescription on the basis of Oxycodone Hydrochloride Sustained-release Tablets.The frequency of outbreak pain and NRS were compared between the two groups.And clinical effect,Karnofsky performance status(KPS)score,adverse reactions as well.Results:Compared with the controlled group,the frequency of outbreak pain and NRS score in the observation group were significantly reduced(P<0.05).Compared with the controlled group(50.00%),the objective response rate(ORR)of the observation group(62.79%)was significantly higher(P<0.05).Compared with before treatment,the KPS score of the two groups increased significantly after radiotherapy,and the increase of the observation group was significantly greater than that of the controlled group,the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Jiedu Dingtong Prescription can not only effectively prevent the pain of LC patients with bone metastases,but also improve the curative effect of radiotherapy,improve the functional status of patients with less adverse reactions,which has high clinical reference value.
