自拟清热化痰活血方预防冠心病支架术后再狭窄临床研究

摘　　要：目的:探讨自拟清热化痰活血方预防冠心病支架术后再狭窄的临床效果。方法:选择2019年9月~2020年9月于我院行冠状动脉内支架植入成功的冠心病患者80例作为研究对象,采用1∶1完全随机对照试验方法将患者分为观察组和对照组。各40例。对照组予以西医常规药物治疗,观察组在对照组治疗基础上加用自拟清热化痰活血方治疗。比较两组术后再狭窄率,治疗后的心电图疗效、中医证候疗效,治疗前后的临床症状积分、超敏C-反应蛋白(hs-CRP)、血浆纤维蛋白原(FIB)水平和血浆粘度,以及治疗期间的用药安全性。结果:治疗后,观察组患者的术后再狭窄率7.50%明显低于对照组的22.50%(P<0.05),且观察组的心电图总有效率和中医证候总有效率均高于对照组(P均<0.05);同时,经治疗3个月后,观察组的临床症状总积分、hs-CRP、FIB水平和血浆粘度均低于对照组(P均<0.05);两组用药前后血、尿、大便常规及肝肾功能均无明显异常。结论:在常规西药治疗基础上采用自拟清热化痰活血方预防冠心病支架术后再狭窄,可明显降低患者术后再狭窄率,改善患者临床症状并减轻炎症反应,提升临床治疗疗效。Objective:To explore the clinical effect of Self-made Qingre Huatan Huoxue Prescription for preventing restenosis after coronary heart disease(CHD)stent implantation.Methods:During the period from September 2019 to September 2020,80 CHD patients who underwent successful coronary artery stent implantation in the hospital were enrolled as research objects.They were divided into an observation group and a controlled group by completely randomized controlled trial method,40 cases in each group.The controlled group was treated with routine western medicine,while the observation group was additionally treated with Self-made Qingre Huatan Huoxue Prescription.The postoperative restenosis rate,curative effect of electrocardiogram(ECG)and TCM syndromes after treatment,scores of clinical symptoms,levels of hypersensitivity C-reactive protein(hs-CRP)and plasma fibrinogen(FIB),plasma viscosity before and after treatment,and medication safety during treatment were compared between the two group.Results:After treatment,postoperative restenosis rate in the observation group was significantly lower than that in the controlled group(7.50%vs 22.50%)(P<0.05),the total response rates of ECG and TCM syndromes were higher than those in the controlled group(P<0.05).After 3 months of treatment,total scores of clinical symptoms,levels of hs-CRP and FIB,and plasma viscosity in the observation group were lower than those in the controlled group(P<0.05).Before and after medication,there were no significant abnormalities in blood,urine and stool routine,and liver-kidney function in either group.Conclusion:The application of Self-made Qingre Huatan Huoxue Prescription on the basis of routine western medicine treatment for preventing restenosis after CHD stent implantation can significantly reduce postoperative restenosis rate,improve clinical symptoms and alleviate inflammation reactions,and improve clinical curative effect.
