叶天士“柴胡劫肝阴,葛根竭胃汁”思想探析

摘　　要：叶天士“柴胡劫肝阴,葛根竭胃汁”一说,在后世医家中多有争议,但大部分医家和学者都是局限于对具体药物或方剂的功效及运用进行探讨。笔者在梳理叶氏医案时发现,此句当为互文之语,“柴胡”“葛根”不能单纯地将其看作是某一味药物或某个方剂,主要是代指风寒之邪初犯人体三阳阶段时所用的辛温散表的治法;“竭胃汁”“劫肝阴”所代表的是人体津液损伤的不同程度,两者有着密切联系,不能截然分开。叶氏此说有着深刻的理法内涵,体现了叶氏手足分治,寒温异法;顾护胃汁,尤重肝阴;重视节令,审察体质等重要的诊治思想。“Bupleurum Robs Liver Yin and Radix Puerariae Exhausts Stomach Juice”of YE Tianshi has been controversial among later generations of physicians,but most physicians and scholars are limited to discussing the efficacy and application of specific drugs or prescriptions.When combing through YE’s medical records,the author found that this sentence should be regarded as an intertextual language.“Bupleurum”and“Pueraria Lobata”cannot be simply regarded as a certain medicine or a certain prescription,but mainly refers to the evil of wind and cold.The treatment of“Pungent and Warm Scattered Exogenous Pathogens”used for the first offense at the stage of human body Sanyang;“Exhausting Stomach Juice”and“Robbing Liver Yin”represent different degrees of human body fluid damage.The two are closely related and cannot be completely separated.YE’s theory has profound connotation of theory and law,which embodies the important diagnosis and treatment thoughts of YE’s separation of hands and feet,cold and temperature differences;protecting stomach juice,especially liver-Yin;paying attention to seasons and examining physique and other important diagnosis and treatment thoughts.
