从《格致余论》谈丁学屏教授对急性痛风性关节炎病因病机的认识及其治疗经验

摘　　要：《格致余论》指出了痛风急性期发作与污浊、血热、外邪三者的相互作用有着直接且密不可分的关系,丁学屏教授与时俱进,认为究其根本是因为脾肾亏虚才导致了污浊在体内生成增多或排泄减少。污浊聚于关节,郁而化热,辛辣酒醴又加剧了血热之势,久病致瘀,进而促成了湿、痰、浊、热、瘀的夹杂为病。在治疗上,丁教授认为急性发病时,重在治标,尤重清热,兼顾余邪,善用古方,复方多用,同时非常注重患者的生活调摄,以求防患未然之效。Gezhi Yulun pointed out that the actue onset of gout had a direct and inseparable relationship with filth,blood heat and external evil.Professor DING Xueping kept pace with the times,he believed that the fundamental reason of gout was deficiency of spleen and kidney.It leaded to the increase of filth or the reduction of excretion.The filth gathered in the joints,which would become hot after a period of time,and the spicy food and alcohol would intensify the blood heat.On this basis,dampness,phlegm,turbidity,heat,and stasis were mixed with each other,which together promoted the occurrence of gout.In terms of treatment,Professor DING believed that in the acute phase,the most important thing was to treat symptoms,especially to clear away heat,in combination with eliminating the remaining pathogens,and made good use of ancient prescriptions and compound prescriptions.At the same time,he attached great importance to the patient’s regulation of living in order to taking preventive measures.
