自拟化浊解毒汤加减治疗慢性萎缩性胃炎的疗效及血清TLR4、IL-4、TNF-α水平变化观察

摘　　要：目的:观察自拟化浊解毒汤加减治疗慢性萎缩性胃炎的疗效及血清Toll样受体4(TLR4)、白细胞介素-4(IL-4)、肿瘤坏死因子-α(TNF-α)水平变化。方法:选取本院2018年10月~2020年10月收治的慢性萎缩性胃炎患者98例,随机数表法分为观察组49例和对照组49例。对照组给予养胃舒胶囊治疗,观察组给予自拟化浊解毒汤加减治疗,并观察两组患者的临床疗效、中医证候积分、胃肠道激素水平和炎性因子水平。结果:观察组治疗总有效率为91.84%,高于对照组的75.51%(P<0.05)。治疗后,两组患者中医证候积分、胃泌素(GAS)、胃动素(MOT)、TNF-α、IL-4、TLR4水平均降低,且观察组低于对照组(P<0.05)。治疗后,两组患者血管活性肠肽(VIP)水平较治疗前升高,且观察组高于对照组(P<0.05)。结论:自拟化浊解毒汤加减治疗慢性萎缩性胃炎的临床疗效佳,且能够改善血清TLR4、IL-4、TNF-α水平。Objective:To observe the curative effect of Self-made Modified Huazhuo Jiedu Decoction on chronic atrophic gastritis,and changes in levels of serum Toll-like receptor 4(TLR4),interleukin-4(IL-4)and tumor necrosis factor-α(TNF-α).Methods:A total of 98 patients with chronic atrophic gastritis admitted to the hospital from October 2018 to October 2020 were enrolled and divided into an observation group(49 cases)and a controlled group(49 cases)by the random number table method.The controlled group was treated with Yangweishu Capsules,while the observation group was treated with Self-made Modified Huazhuo Jiedu Decoction.The clinical curative effect,scores of TCM syndromes,levels of gastrointestinal hormones and inflammatory factors in both groups were observed.Results:The total response rate of treatment in the observation group was higher than that in the controlled group(91.84%vs 75.51%)(P<0.05).After treatment,scores of TCM syndromes,levels of gastrin(GAS),motilin(MOT),TNF-α,IL-4 and TLR4 in both groups were decreased,which were lower in the observation group than the controlled group(P<0.05).After treatment,level of vasoactive intestinal peptide(VIP)in both groups was increased,which was higher in the observation group than the controlled group(P<0.05).Conclusion:The clinical curative effect of Self-made Modified Huazhuo Jiedu Decoction is good on chronic atrophic gastritis,which can improve levels of serum TLR4,IL-4 and TNF-α.
