连蒲清热燥湿方治疗消化性溃疡湿热蕴结证对患者血清胃泌素、白细胞介素-17水平的影响

摘　　要：目的:观察连蒲清热燥湿方治疗消化性溃疡湿热蕴结证对患者血清胃泌素(GAS)、白细胞介素-17(IL-17)水平的影响。方法:将我院2017.2~2019.12期间118例消化性溃疡湿热蕴结证患者采用随机数字表法分为观察组、对照组各59例,两组患者均予以常规西药治疗,观察组在此基础上辅以连蒲清热燥湿方治疗。比较两组患者治疗2个月后临床疗效,治疗前、治疗2个月后中医证候积分、消化道激素[胃动素(MTL)、胃泌素(GAS)]、血清炎症因子[一氧化氮(NO)、白细胞介素-17(IL-17)]水平变化,及治疗2个月幽门螺杆菌(Hp)根除率、随访6个月复发率。结果:治疗2个月后,观察组治疗总有效率明显高于对照组(P<0.05);两组上腹痛、嗳气、痞满、反酸四大主症积分均较治疗前降低,且观察组低于对照组(P<0.05);两组MTL、GAS水平均较治疗前降低,且观察组低于对照组(P<0.05);两组NO、IL-17水平均较治疗前降低,且观察组低于对照组(P<0.05);治疗2个月后,观察组Hp清除率高于对照组,随访6个月复发率低于对照组(P<0.05)。结论:连蒲清热燥湿方治疗消化性溃疡湿热蕴结证疗效确切,有助于改善临床症状,抑制胃酸分泌,提高Hp清除率,减少复发。Objective:To observe the effect of Lianpu Qingre Zaoshi Decotion on levels of serum gastrin(GAS)and interleukin-17(IL-17)for patients with peptic ulcer of dampness-heat accumulation symdrome.Methods:118 patients with peptic ulcer of dampness-heat accumulation symdrome in our hospital from February 2017 to December 2019 were divided into an observation group and a controlled group by the random number table method,with 59 cases in each group.The two groups were treated with conventional western medicine,and the observation group was supplemented with Lianpu Qingre Zaoshi Decotion on this basis.The clinical efficacy after 2 months of treatment,as well as TCM syndromes scores,gastrointestinal hormones[motilin(MTL),gastrin(GAS)]and serum inflammatory factors[nitric oxide(NO),interleukin-17(IL-17)]before treatment and after 2 months of treatment,Helicobacter pylori(Hp)clearance rate after 8 weeks of treatment and recurrence rate after 6 months of follow-up were compared between the two groups.Results:After 2 months of treatment,the total effective rate of treatment in the observation group was significantly higher than that in the controlled group(P<0.05).The scores of four main symptoms of upper abdominal pain,belching,distention and fullness and acid reflux in the two groups were decreased compared with those before treatment,and the scores in the observation group were lower than those in the controlled group(P<0.05).The levels of MTL and GAS in the two groups were decreased compared with those before treatment,and the levels in the observation group were lower than those in the controlled group(P<0.05).The levels of NO and IL-17 in the two groups were decreased compared with those before treatment,and the levels in the observation group were lower than those in the controlled group(P<0.05).The Hp clearance rate in the observation group after 2 months of treatment was higher than that in the controlled group while the recurrence rate after 2 months of follow-up was lower than that in the controlled group(P<0.05).Co
