高血压病患者中医证型分布及其与心血管易损性的相关性探究

摘　　要：目的:探究高血压病患者中医证型分布及与心血管易损性的相关性。方法:以2019年3月~2021年7月103例原发性高血压患者为研究对象,根据中医证型的不同分为肝火亢盛型(n=29)、痰湿壅盛型(n=51)、阴阳两虚型(n=25)、阴虚阳亢型(n=8),比较四组患者的一般资料、高血压分级、心血管危险分层及心血管易损因子水平,分析患者高血压分级及心血管分级与心血管易损因子的相关性。结果:四种证型患者的性别、年龄等对比无明显差异(P>0.05);阴阳两虚型患者的病程长于其他三种证型(P<0.05);不同证型患者的高血压分级对比无明显差异(P>0.05),阴阳两虚型患者的心血管危险分层高于肝火亢盛型、痰湿壅盛型(P<0.05);不同证型患者的同型半胱氨酸(Hcy)水平对比无明显差异(P>0.05),阴阳两虚型患者的脑钠肽(BNP)、超敏-C反应蛋白(hs-CRP)水平高于其他三种证型,血管性血友病因子(VWF)水平高于肝火亢盛型、阴虚阳亢型(P<0.05);患者hs-CRP、VWF、BNP与心血管分级呈正相关(P<0.05),患者心血管易损因子与高血压分级无相关性(P>0.05)。结论:阴阳两虚型高血压患者的心血管危险程度及心血管易损性较高,且患者心血管易损性与心血管分级相关。Objective:To explore the distribution of TCM syndromes in patients with hypertension and their correlation with cardiovascular vulnerability.Methods:A total of 103 patients with essential hypertension were enrolled as the research objects between March 2019 and July 2021.According to different TCM syndromes,they were divided into liver-fire hyperactivity type(n=29),phlegm-dampness flourishing type(n=51),Yin-Yang deficiency type(n=25)and Yin-deficiency Yang hyperactivity type(n=8).The general data,hypertension grading,cardiovascular risk stratification and levels of cardiovascular vulnerability factors among the four groups were compared.The correlation between hypertension grading,cardiovascular grading and cardiovascular vulnerability factors was analyzed.Results:There was no significant difference in gender or age among the four groups(P>0.05).The disease course in patients with Yin-Yang deficiency type was longer than that with other syndromes(P<0.05).There was no significant difference in hypertension grading among patients with different syndromes(P>0.05).The cardiovascular risk stratification in patients with Yin-Yang deficiency type was higher than that with liver-fire hyperactivity type and phlegm-dampness flourishing type(P<0.05).There was no significant difference in level of homocysteine(Hcy)among patients with different syndromes(P>0.05).The levels of brain natriuretic peptide(BNP)and high-sensitivity-C-reactive protein(hs-CRP)in patients with Yin-Yang deficiency type were higher than those with other syndromes,and level of von Willebrand factor(VWF)was higher than that with liver-fire hyperactivity type and Yin-deficiency Yang hyperactivity type(P<0.05).The hs-CRP,VWF and BNP were positively correlated with cardiovascular grading(P<0.05).The cardiovascular vulnerability factors were not correlated with hypertension grading(P>0.05).Conclusion:The cardiovascular risk and cardiovascular vulnerability are relatively higher in patients with hypertension of Yin-Yang deficiency type.The cardiovascular vulne
