养血扶正膏对乳腺癌CAF方案化疗所致骨髓抑制患者外周血象及免疫功能的影响

摘　　要：目的:探究养血扶正膏对乳腺癌CAF化疗方案所致骨髓抑制患者外周血象及免疫功能的影响。方法:选取苏州大学附属第二医院(核工业总医院)2018年4月~2021年5月收治的90例乳腺癌CAF化疗方案后骨髓抑制患者作为研究对象。以电脑随机数法将其分为两组,对照组45例予以粒细胞-巨噬细胞集落刺激因子(G-CSF)支持治疗及血液成分输注治疗,观察组45例在G-CSF支持治疗基础上联合口服养血扶正膏。比较两组患者治疗前后中医证候积分、外周血象指标、免疫学指标[自然杀伤细胞(NK)、抑制T细胞(Ts)、辅助T细胞(Th)]、肿瘤标志物[糖类抗原125(CA125)、CA153及癌胚抗原(CEA)]及不良反应发生率。结果:治疗后,两组患者的中医证候积分均低于治疗前,且观察组低于对照组(P<0.05);治疗后,两组患者的白细胞、血小板及血红蛋白水平均高于治疗前,且观察组高于对照组(P<0.05);治疗后两组患者的NK、Ts及Th水平均高于治疗前,且观察组高于对照组(P<0.05);治疗后两组患者的CA125、CA153及CEA水平均低于治疗前,且观察组低于对照组(P<0.05);两组不良反应发生率比较差异无统计学意义(P>0.05)。结论:养血扶正膏辅疗乳腺癌CAF化疗方案所致的骨髓抑制,可有效改善患者临床病症,恢复患者外周血象水平及免疫功能,降低肿瘤标志物水平且安全性良好。Objective:To explore the effect of Yangxue Fuzheng Ointment on peripheral blood and immune function for patients with myelosuppression induced by CAF chemotherapy.Methods:A total of 91 patients with myelosuppression after CAF chemotherapy were selected from The Second Hospital Affiliated to Suzhou University from April 2018 to May 2021.They were divided into two groups by computer random number method.45 cases in the controlled group were treated with granulocyte macrophage colony stimulating factor(G-CSF)support and blood component infusion,and 45 cases in the observation group were treated with Yangxue Fuzheng Ointment on the basis of G-CSF support.The TCM syndrome scores,peripheral hemogram indexes and immunological indexes[natural killer cells(NK)]were compared between the two groups before and after treatment.Results:After treatment,the TCM syndrome scores of the two groups were lower than those before treatment,and the observation group was lower than that of the controlled group(P<0.05).After treatment,the levels of leukocytes,platelets and hemoglobin in the two groups were higher than those before treatment,and those in the observation group were higher than those in the controlled group(P<0.05);after treatment,the levels of NK,TS and Th in the two groups were higher than those before treatment,and those in the observation group were higher than those in the controlled group(P<0.05);after treatment,the levels of CA125,CA153 and CEA in the two groups were lower than those before treatment,and those in the observation group were lower than those in the controlled group(P<0.05);there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Yangxue Fuzheng Ointment is effective in treating myelosuppression induced by CAF chemotherapy for breast cancer,which can effectively improve the clinical symptoms,restore the peripheral blood level and immune function of patients,reduce the level of tumor markers and is safe.
