活血消肿汤联合快速康复训练对老年全膝关节置换术(TKA)后患者运动功能及炎症反应的影响

摘　　要：目的:观察活血消肿汤联合快速康复训练对老年全膝关节置换术(TKA)后患者运动功能及炎症反应的影响。方法:选取2018年8月~2020年2月北京中医医院顺义医院收治的80例重度膝关节骨性关节炎(KOA)患者,均行单侧TKA治疗,根据乱数表法均分为对照组和观察组,对照组术后接受快速康复训练,观察组术后接受活血消肿汤联合快速康复训练,对比两组患者术后当天及术后14d髌骨上下3寸肿胀值、膝关节最大屈曲和伸直角度、膝关节功能、下肢运动功能、炎症因子水平及不良反应发生情况。结果:术后14天观察组髌骨上下3寸肿胀值、膝关节最大伸直角度、血清血沉(ESR)和C反应蛋白(CRP)水平均明显低于对照组,膝关节最大屈曲角度和美国特种外科医院(HSS)、Fugl-Meyer评测法(FMA)评分明显高于对照组(P<0.05);两组患者均未出现不良反应。结论:活血消肿汤联合快速康复训练可有效消除术后患膝肿胀,恢复关节活动度,提升老年TKA后患者运动功能,改善炎症因子水平,且安全性高,有利于患者早期功能康复,值得推广。Objective:To explore the effect of Huoxue Xiaozhong Decoction combined with rapid rehabilitation training on motor function and inflammation for elderly patients after total knee arthroplasty(TKA).Methods:A total of 80 severe knee osteoarthritis(KOA)admitted to our hospital(from August 2018 to February 2020)were selected and treated with unilateral TKA.According to the random number table method,they were divided into two groups,the controlled group(n=40)received rapid rehabilitation training,the treatment group(n=40)received Huoxue Xiaozhong Decoction combined with rapid rehabilitation training after the operation,and the swelling value of 3 inches above and below the patella and the maximum knee flexion were compared on the day and 14 days after the operation.And extension angle,knee joint function,lower limb motor function,inflammatory factor levels and adverse reactions.Results:14 days postoperatively,the swelling value of the upper and lower 3 inches of the patella,the maximum knee extension angle,serum erythrocyte sedimentation rate(ESR)and C-reactive protein(CRP)levels in the treatment group were significantly lower than those in the controlled group.The maximum knee flexion angle,Hospital for Special Surgery(HSS)and Fugl-Meyer Assessment(FMA)scores were significantly higher than those in the controlled group(P<0.05).There were no adverse reactions in both groups.Conclusion:Huoxue Xiaozhong Decoction combined with rapid rehabilitation training can effectively eliminate postoperative knee swelling,restore joint mobility,improve motor function of elderly patients after TKA,improve the level of inflammatory factors,and have high safety,which is beneficial to early functional rehabilitation of patients and is worthy of promotion.
