乌梅丸加减联合三联疗法治疗老年HP阳性慢性萎缩性胃炎对患者胃动力及白介素族的影响

摘　　要：目的:观察乌梅丸汤剂加减联合三联疗法治疗老年幽门螺杆菌(Hp)阳性慢性萎缩性胃炎对患者胃动力及白介素族的影响,为临床治疗提供参考。方法:选取我院2017年10月~2020年5月收治的老年HP阳性慢性萎缩性胃炎患者140例,分为两组,每组70例。对照组给予三联疗法治疗,观察组在对照组基础上加用乌梅丸汤剂加减治疗。记录两组治疗前后中医症候积分的变化,检测两组患者治疗前后胃动力指标、白介素族因子的变化,并统计两组Hp根除率和治疗期间的不良反应。结果:治疗后两组胃脘胀痛、纳差、不思饮食、口渴喜饮、胸闷恶心及乏力积分逐渐下降(P<0.05),进一步组间比较发现,观察组各项中医症候积分低于对照组(P<0.05)。治疗后两组餐前和餐后胃电图幅值均逐渐上升(P<0.05),进一步组间比较发现,观察组胃动力高于对照组(P<0.05)。治疗后两组白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、白细胞介素-32(IL-32)逐渐下降(P<0.05),进一步组间比较发现,观察组白介素族水平低于对照组(P<0.05)。治疗后,观察组的Hp根除率为94.29%,高于对照组的82.86%,组间比较发现,有统计学意义(P<0.05),不良反应率为5.71%,与对照组的7.14%组间比较发现,无统计学意义(P<0.05)。结论:乌梅丸汤剂加减联合三联疗法治疗老年HP阳性慢性萎缩性胃炎可降低炎症因子的表达,改善胃动力,提高Hp根除率。Objective:Effect of using Modified Wumei Pill combined with triple therapy on gastric motility and interleukin group in the treatment of elderly patients with Helicobacter pylori(Hp)positive chronic atrophic gastritis were observed,which provided reference for clinical treatment.Methods:Selecting 140 elderly HP positive chronic atrophic gastritis patients in our hospital(from October 2017 to May 2020),divided into 2 groups according to the treatment method,70 cases in each group.The controlled group was treated with triple therapy,and the observation group was treated with Modified Wumei Pill on the basis of the controlled group.To record the changes of TCM symptom score before and after treatment,to detect the changes of gastric motility index and interleukin group factors before and after treatment,and counted 2 groups Hp eradication rate and adverse reactions during treatment.Results:The scores of epigastric distending pain,anorexia,diet,thirst and drink,chest tightness,nausea and fatigue decreased gradually in the two groups after treatment(P<0.05).The amplitude of gastroscopy increased gradually(P<0.05)in the two groups after treatment,and the gastric motility in the observation group was higher than that in the controlled group(P<0.05).The interleukin-1β(IL-1β),interleukin-6(IL-6),interleukin-8(IL-8)and interleukin-32(IL-32)decreased gradually in the two groups after treatment(P<0.05).Further inter-group comparison revealed that the level of the interleukin family in the observation group was lower than that in the controlled group(P<0.05).The Hp eradication rate of the observation group was 94.29%,higher than that of the controlled group 82.86%.The comparison between the two groups found that there was statistical significance(P<0.05)and the adverse reaction rate was 5.71%.Compared with the controlled group of 7.14%,there was no statistical significance(P<0.05).Conclusion:Using Wumei Pill combined with triple therapy in the treatment of elderly HP positive chronic atrophic gastritis can reduce the expres
