柴胡加龙骨牡蛎汤加减治疗心脏神经官能症患者的中医证候、不良情绪和安全性临床研究

摘　　要：目的:观察柴胡加龙骨牡蛎汤加减治疗心脏神经官能症(CN)患者的中医证候、不良情绪和安全性。方法:选取2016年6月~2020年1月杨凌示范区医院收治的80例CN患者,根据随机数表法分为2组,对照组(n=40)接受常规治疗,观察组(n=40)接受柴胡加龙骨牡蛎汤加减治疗,4周后,对比两组患者临床疗效和治疗前后中医症状积分、不良情绪、躯体化症状、心电图变化及药物不良反应,随访3个月,对比复发率。结果:观察组临床疗效和心电图变化明显高于对照组(P<0.05);治疗后两组心悸、胸闷、心烦易怒、失眠多梦、气短积分和汉密尔顿焦虑量表(HAMD)、汉密尔顿抑郁量表(HAMA)、躯体化症状自评量表(SSS)评分均明显降低,观察组改善更明显(P<0.05);两组均未出现严重不良反应,随访3个月,观察组复发率明显低于对照组(P<0.05)。结论:柴胡加龙骨牡蛎汤加减治疗CN疗效确切,可有效改善中医证候、心电图及不良情绪,缓解躯体化症状,且安全性高,复发率低,值得推广。Objective:To explore the TCM syndromes,unhealthy emotions and safety of patients with cardiac neurosis(CN)treated by Chaihu plus Longgu Muli Decoction.Methods:80 CN patients admitted to our hospital(June 2016 to January 2020)were selected and divided into 2 groups according to the random number table method.The controlled group(n=40)were treated with conventional treatment and the observation group(n=40)were treated Chaihu plus Longgu Muli Decoction,4 weeks later,compared the clinical efficacy,TCM symptom scores,unhealthy emotions,somatization symptoms,electrocardiogram change and adverse reactions of the two groups of patients before and after treatment,followed up for 3 months,compared the recurrence rate.Results:The clinical efficacy and electrocardiogram change of the observation group were significantly higher than those of the controlled group(P<0.05).The scores of palpitation,chest distress,upset irritability,insomnia and dreaminess,shortness of breath,HAMD,HAMA and SSS were significantly reduced,and the observation group improved more significantly(P<0.05);no serious adverse reactions occurred in both groups,followed up for 3 months,the recurrence rate of the observation group was significantly lower than the controlled group(P<0.05).Conclusion:Chaihu plus Longgu Muli Decoction is effective in treating CN.It can effectively improve TCM syndromes,electrocardiogram and unhealthy emotions,relieve somatization symptoms,and has high safety and low recurrence rate.It is worthy of promotion.
