摘 要:目的:对比分析多层螺旋CT扫描、高千伏DR在职业性肺病筛查中的应用价值。方法:地点选择云南省职业病医院,回顾性纳入具有明确职业性粉尘接触史的患者80例进行研究,2021.1开始,2023.10结束,统一接受多层螺旋CT扫描、高千伏DR检查,对照职业病诊断机构作出的职业病诊断,对比不同方法分期结果、胸片质量以及诊断差异。结果:(1)经对比,两组分期结果无明显差异,P>0.05。(2)在此次检验中,高千伏DR一级片50张,一级片率为62.50%,二级片13张,二级片率为16.25%,总有效率78.75%;多层螺旋CT片52张,一级片率为65.00%,二级片15张,二级片率为18.75%,总有效率83.75%。两组胸片质量对比无明显差异,P>0.05。(3)经对比,高千伏DR和多层螺旋CT在总体密集度、肺区数、小阴影聚集/大阴影、合并症数、诊断分期方面对比无明显差异,P>0.05。结论:单一应用多层螺旋CT扫描或高千伏DR局限性较大,故笔者建议将二者有机结合,联合用于职业性肺病筛查中,以进一步提高诊断价值。Objective:To compare and analyze the application value of multi-slice spiral CT scanning and high kV DR in screening occupational lung diseases.Methods:Selecting the Occupational Disease Hospital of Yunnan Province,80 patients with a clear history of occupational dust exposure were enrolled in the study retrospectively.From January,2021 to October,2023,they all received multi-slice spiral CT scanning and high kV DR examination.Compared with the occupational disease diagnosis made by occupational disease diagnosis institutions,the staging results,chest X-ray quality and diagnostic differences of different methods were compared.Results:(1)By comparison,there was no significant difference in staging results between the two groups(P>0.05).(2)In this inspection,there are 50 first-class films with high kV DR,the first-class film rate is 62.50%,and the second-class film rate is 16.25%,with a total effective rate of 78.75%;There were 52 multi-slice spiral CT films,the first-class film rate was 65.00%,the second-class film rate was 18.75%,and the total effective rate was 83.75%.There was no significant difference in the quality of chest films between the two groups(P>0.05).(3)By comparison,there was no significant difference between high kV DR and multi-slice spiral CT in the overall density,the number of lung regions,the small shadow gathering/large shadow,the number of complications and the diagnosis stages(P>0.05).Conclusion:Single application of multi-slice spiral CT scanning or high kV DR has great limitations,so the author suggests that they should be combined organically and used in occupational lung disease screening to further improve the diagnostic value.
