岭南医家何梦瑶《医碥》治痹学术思想探析

摘　　要：总结清代岭南名医何梦瑶所撰《医碥》中治疗痹证的学术思想。何梦瑶论痹以《黄帝内经》为圭臬,对痹证的分类、命名以及病因病机皆继承了《黄帝内经》中的论述,认为风、寒、湿邪阻遏,气机不利是痹证的基本病机。同时,何梦瑶提出,导致气机阻遏的主要因素是寒、湿二气;痹证的发生与经络受阻及自身元气的盛衰密切相关,并从气之盛衰与寒湿之邪关系论证痹证中痛、麻证候的成因。治疗上,确立以通为用、益气扶正的治疗方法,选方用药擅以辛温之药通络而调畅气机,以酒为引,行经络而通痹塞,温血脉而散凝瘀。何梦瑶治痹思想可为现代中医临床治疗痹证提供思路。This article summarized the academic thoughts for the treatment of arthralgia syndrome recorded in Yi Bian(Medical Stepping Stone for Practitioners)written by Lingnan celebrated practitioner HE Meng-Yao of the Qing Dynasty.HE Meng-Yao suggested that the classification,nomenclature,etiology and pathogenesis of arthralgia syndrome should follow the descriptions recorded in Huangdi’s Canon of Medicine,which is the guideline of the diagnosis and treatment of arthralgia syndrome.HE Meng-Yao believes that the basic pathogenesis of arthralgia syndrome is due to the disturbance of qi movement resulting from the blockage of wind,cold,and pathogenic damp,and the primary factors of disturbance of qi movement are the pathogens of cold and dampness.The occurrence of arthralgia syndrome is closely related with the obstruction of meridians and collaterals together with the consumption of original qi.From the aspect of the relationship of consumption of original qi with the pathogens of cold and dampness,HE Meng-Yao analyzed the development of pain and numbness in arthralgia syndrome.In terms of the treatment,the therapeutic principles of functioning well when unobstructed and replenishing qi to support healthy qi were established,and the therapeutic methods were performed through the medication with pungent-warm drugs to unblock collaterals and regulate qi movement,also by using alcohol as an extra conducting ingredient to promote meridians and collaterals for clear the blockages,and to warm the blood vessel for resolving stagnation and stasis.HE Meng-Yao’s thoughts for the treatment of arthralgia syndrome may provide new ideas for the modern clinical treatment of arthralgia syndrome with traditional Chinese medicine.
