五苓散在眼科病症黄斑水肿临证中的应用

摘　　要：五苓散出自张仲景《伤寒论》,由猪苓、白术、茯苓、泽泻与桂枝组成,主治太阳蓄水证,有利水渗湿、温阳化气之功效。黄斑水肿多因中心性浆液性脉络膜视网膜病变、糖尿病性视网膜病变、视网膜静脉阻塞、术后眼内炎症等所致,其中医病机主要为脾虚水湿内停,上犯于目,治疗需健脾益气,利水渗湿。该文通过列举典型案例,对五苓散治疗各类型黄斑水肿进行总结,以期为五苓散在现代眼科临证中的应用提供思路。Wuling Powder is recorded in Treatise on Exogenous Febrile Diseases written by ZHANG Zhong-Jing.The prescription is composed of Polyporus,Rhizoma Atractylodis Macrocephalae,Poria,Rhizoma Alismatis and Ramulus Cinnamomi,has the actions of inducing diuresis and draining dampness,warming yang to transform into qi,and is indicated for taiyang water-retention syndrome.Macular edema is mainly caused by central serous chorioretinopathy,diabetic retinopathy,retinal vein occlusion,postoperative ocular inflammation,etc.The pathogenesis of macular edema is due to spleen deficiency with internal retention of water-dampness,and the upward attack of the eyes by the water-dampness.Therefore,the treatment of macular edema should focus on strengthening the spleen and replenishing qi,inducing diuresis and draining dampness.In this article,proven cases were presented to illustrate the application of Wuling Powder in the treatment of various types of macular edema,and it is expected to supply thoughts for the application of Wuling Powder in the treatment of eye diseases of modern ophthalmology.
