伤科接骨片联合经皮椎体成形术治疗骨质疏松性椎体压缩性骨折临床研究

摘　　要：目的:观察伤科接骨片联合经皮椎体成形术(PVP)治疗骨质疏松性椎体压缩性骨折(OVCF)的临床效果。方法:采用随机数字表法将104例OVCF患者分为对照组、观察组各52例。对照组给予PVP治疗,观察组采用伤科接骨片联合PVP治疗。比较2组治疗前及治疗1周、3个月后视觉模拟评分法(VAS)评分、Cobb角、腰椎骨密度、中医证候积分及血清骨钙素(BGP)、Ⅰ型胶原羧基端肽β特殊序列(β-CTX)水平。结果:治疗1周、3个月后,2组VAS评分、Cobb角、中医证候积分均较治疗前降低(P<0.05),观察组治疗后VAS评分、中医证候积分均低于对照组(P<0.05),但2组治疗后Cobb角比较,差异无统计学意义(P>0.05)。治疗3个月后,2组血清BGP水平均较治疗前升高(P<0.05),β-CTX水平降低(P<0.05),观察组治疗3个月后血清BGP水平高于对照组(P<0.05),β-CTX水平低于对照组(P<0.05)。2组治疗前后腰椎骨密度比较,差异无统计学意义(P>0.05)。结论:伤科接骨片联合PVP治疗OVCF,能够有效减轻患者疼痛症状,改善患者骨代谢功能。Objective:To observe the clinical effect of the therapy of Shangke Jiegu Tablets combined with percutaneous vertebroplasty(PVP)on osteoporotic vertebral compression fracture(OVCF).Methods:A total of 104 OVCF patients were divided into the control group and the observation group,with 52 cases in each group.The control group was treated with PVP,and the observation group was treated with Shangke Jiegu Tablets combined with PVP.The scores of Visual Analogue Scale(VAS),Cobb angle,lumbar vertebrae bone density,traditional Chinese medicine(TCM)syndrome and levels of serum bone gammacarboxyglutamic-acid-containing proteins(BGP)andβcross-linked C-telopeptide of typeⅠcollagen(β-CTX)were compared between the two groups before treatment and one week and three months after treatment.Results:After one week and three months of treatment,the VAS scores,Cobb angle and TCM syndrome scores in the two groups were decreased when compared with those before treatment(P<0.05).After treatment,the VAS scores and TCM syndrome scores in the observation group were lower than those in the control group(P<0.05),but there was no significant difference in the comparison of Cobb angle between the two groups after treatment(P>0.05).After three months of treatment,the serum BGP levels in both groups were increased(P<0.05),and theβ-CTX levels were decreased(P<0.05).After three months of treatment,the serum BGP level in the observation group was higher than that in the control group(P<0.05),and theβ-CTX level was lower than that in the control group(P<0.05).There was no significant difference in the comparison of lumbar vertebra bone density between the two groups before and after treatment(P>0.05).Conclusion:The therapy of Shangke Jiegu Tablets combined with PVP can effectively reduce pain symptoms and improve bone metabolism.
