摘  要:目的探究血清淀粉样蛋白A(SAA)联合C反应蛋白(CRP)、白细胞(WBC)计数检测在呼吸道感染患儿中的临床意义。方法选择2019年6月-2021年5月在广汉市人民医院住院的呼吸道感染患儿180例,根据患儿的疾病类型分为细菌组98例,病毒组82例,选择同期来本院检查的健康儿童60名作为健康组,比较三组的SAA、CRP、WBC水平,并采用受试者工作特征(ROC)曲线分析SAA、CRP、WBC对细菌感染或病毒感染患儿的预测价值。比较不同病毒组[EB病毒(EBV)组53例、巨细胞病毒(CMV)12例、单纯疱疹病毒(HSV)17例]的SAA、CRP、WBC水平。结果三组SAA、CRP、WBC比较,差异有统计学意义(F=2123.811、3075.530、241.073,P均<0.05);细菌组的SAA、CRP、WBC水平高于病毒组及健康组,差异有统计学意义(P<0.05);病毒组的SAA、CRP水平高于健康组,差异有统计学意义(P<0.05);SAA、CRP与WBC均成正相关(r=0.647、0.619,P均<0.05);经ROC曲线分析,SAA、CRP、WBC水平区别细菌感染与病毒感染时的AUC分别为0.937、0.915、0.883;3种不同病毒感染时,SAA、CRP水平比较,差异有统计学意义(F=57.929、57.158,P均<0.05);WBC水平比较,差异无统计学意义(F=0.111,P>0.05)。结论SAA、CRP、WBC水平可为临床诊断住院儿童呼吸道感染类型提供有效参考。Objective To explore the clinical significance of serum amyloid A(SAA)combined with C-reactive protein(CRP)and white blood cell count(WBC)detection in respiratory tract infection children.MethodsA total of 180children with respiratory tract infection admitted to the hospital were enrolled between June 2019 and May 2021.Accordingto disease types,they were divided into bacterial group(98 cases)and viral group(82 cases).Sixty healthy childrenundergoing physical examination during the same period were enrolled as healthy group.SAA,CRP and WBC among thethree groups were compared.The predictive value of SAA,CRP and WBC for bacterial or viral infection was analyzed byreceiver operating characteristic(ROC)curves.The levels of SAA,CRP and WBC in different virus groups[53 cases withEB Virus(EBV),12 cases with cytomegalovirus(CMV),17 cases with herpes simplex virus(HSV)]were compared.ResultsThe differences in SAA,CRP and WBC among the three groups were statistically significant(F=2123.811,3075.530,241.073,all P<0.05).The levels of SAA,CRP and WBC were the highest in bacterial group,followed by viralgroup and healthy group(P<0.05).The levels of SAA,CRP in viral group were higher than those in healthy group(P<0.05).SAA,CRP and WBC were positively correlated(r=0.647,0.619,all P<0.05).ROC curve analysis showed that AUCvalues of SAA,CRP and WBC for distinguishing bacterial infection and viral infection were 0.937,0.915 and 0.883,respectively.The differences in SAA and CRP among patients with the three kinds of virus infection were statisticallysignificant(F=57.929,57.158,all P<0.05),while the difference in WBC level was not statistically significant(F=0.111,P>0.05).ConclusionThe levels of SAA,CRP and WBC could provide effective reference for the clinical diagnosis ofrespiratory tract infection in hospitalized children.
