系统性红斑狼疮并发粪类圆线虫感染1例

摘　　要：本文报道1例因系统性红斑狼疮接受激素联合免疫抑制剂治疗3月余,以腹泻为首发症状就诊,继而出现全身瘀斑、胸闷气喘及发热等临床表现,予积极控制基础病及抗感染治疗后效果不佳,迅速发展为感染性休克、多器官功能衰竭而死亡,最终在支气管肺泡灌洗液标本中检出粪类圆线虫而明确诊断,提示粪类圆线虫感染对免疫功能低下者存在高危致死风险,早诊断、早治疗对决定预后极为重要。This article reported a case of systemic lupus erythematosus treated with hormone combined with immunosupprestant for 3 months.Diarrhea was the first symptom,followed by systemic ecchymosis,chest tightness,asthma,fever and other clinical manifestation.After active control of the disease and anti-infection treatment,the disease developed rapidly into septic shock,multiple organ failure and death;finally,the diagnosis was confirmed by the detection of nematode in bronchoalveolar lavage fluid.It suggested that Strongyloides stercoralis infection could increase the risk of death for people with low immune function.Early diagnosis and treatment were extremely important in determining prognosis.
