摘　　要：目的探讨Wnt细胞信号转导蛋白在不同Wagner分级糖尿病足溃疡(DFU)患者中的表达水平及意义。方法选取汕头潮南民生医院2020年1月-2021年1月收治的136例DFU患者为研究组,根据Wagner分级不同分为Ⅰ级组(n=28)、Ⅱ级组(n=29)、Ⅲ级组(n=36)、Ⅳ级组(n=26)、Ⅴ级组(n=17)。另选取同期非DFU创面患者20例为对照组。采用免疫组织化学染色法、RT-PCR、Western bolt法分别检测各组患者创面新鲜肉芽组织中β-联环素(β-catenin)、糖原合成激酶-3β(GSK-3β)表达水平,采用Spearman相关性分析不同Wagner分级与β-catenin、GSK-3β水平的关系。结果对照组、Ⅰ级组、Ⅱ级组、Ⅲ级组、Ⅳ级组、Ⅴ级组β-catenin表达OD值依次降低,GSK-3β表达OD值依次升高,差异有统计学意义(P<0.05),且两两组间比较差异有统计学意义(P<0.05)。对照组、Ⅰ级组、Ⅱ级组、Ⅲ级组、Ⅳ级组、Ⅴ级组β-catenin mRNA表达水平依次降低,GSK-3βmRNA表达水平依次升高,差异有统计学意义(P<0.05),两两组间比较差异有统计学意义(P<0.05)。Wagner分级与β-catenin水平成负相关(r=-0.863,P<0.001),与GSK-3β水平成正相关(r=0.749,P<0.001)。结论Wagner分级与Wnt细胞信号调控因子β-catenin、GSK-3β水平具有显著相关性,激活Wnt/β-catenin信号通路可能对DFU患者产生积极影响。Objective To investigate the expression and significance of Wnt signaling pathway related protein in diabetic foot ulcer(DFU)patients with different Wagner grades.Methods A total of 136 patients with DFU and 20 patients with non-DFU wounds in Shantou Chaonan Minsheng Hospital from January 2020 to January 2021 were enrolled,and set as study group and control group,respectively.Study group was further divided into 5 groups according to different Wagner grades,grade I group(n=28),gradeⅡgroup(n=29),gradeⅢgroup(n=36),gradeⅣgroup(n=26)and grade V group(n=17).Immunohistochemical staining,RT-PCR,and Western bolt methods were used to detect the expression levels ofβ-catenin and glycogen synthesis kinase-3β(GSK-3β).Spearman correlation was used to analyze the relationship between different Wagner grades andβ-catenin and GSK-3βexpression levels.Results Among the control group,grade I group,gradeⅡgroup,gradeⅢgroup,gradeⅣgroup and grade V group,there was a decrease trend in OD value ofβ-catenin,and an increase trend in OD value of GSK-3β(all P<0.05);meanwhile,a decrease trend was found inβ-catenin m RNA expression level,and an increase trend was found in GSK-3βm RNA expression level among those groups(all P<0.05).Furthermore,the paired comparison also showed statistical difference(all P<0.05).Wagner grade classification was negatively correlated withβ-catenin level(r=-0.863,P<0.001),and positively correlated with GSK-3βlevel(r=0.749,P<0.001).Conclusion Wagner grade classification has certain correlation with the levels of Wnt cell signaling regulatory factorsβ-catenin and GSK-3β,and the activation of the Wnt/β-catenin signaling pathway may play a positive role in recovery of DFU.
