摘　　要：目的检测Toll样受体9(TLR9)基因多态性,了解其与脓毒症易感性的关系,并探讨其临床意义。方法收集2018年10月-2020年10月创伤和胰腺炎患者临床资料,其中发生脓毒症173例为研究组,未发生脓毒症236例作为对照组。检测TLR9(rs187084、rs743836、rs352139、rs352140)4个位点基因多态性,使用logistic分析TLR9基因多态性影响脓毒症的危险因素。结果173例脓毒症患者中,内科患者106例(61.27%),外科患者67例(38.73%)。TLR9位点基因多态性进行拟合优度χ^(2)检验,在研究组和对照组中均符合Hardy-Weinberg遗传平衡检验(P>0.05)。两组rs187084、rs352139、rs352140位点基因型和等位基因比较,差异无统计学意义(P>0.05),而两组rs743836多态位点基因型和等位基因比较,差异有统计学意义(P<0.05)。多因素logistic分析影响脓毒症的独立危险因素结果显示,rs743836多态位点是影响脓毒症的独立危险因素(P<0.05)。结论TLR9 rs743836位点基因多态性的创伤和胰腺炎患者易发生脓毒症风险。Objective To detect toll like receptor 9(TLR9)gene polymorphism,understand its relationship with sepsis susceptibility,and explore its clinical significance.Methods From October 2018 to October 2020,173 patients with trauma and pancreatitis developed sepsis(study group)and 236 patients without sepsis(control group)were enrolled for the study.The gene polymorphism of TLR9(rs187084,rs743836,rs352139,rs352140)was detected,and the risk factors of sepsis were analyzed by logistic regression.Results There were 173 sepsis patients,106 internal medicine patients(61.27%),and 67 surgical patients(38.73%).TLR9 gene polymorphism was tested by goodness-of-fitχ^(2) test,and met the Hardy-Weinberg genetic balance test in both sepsis patients and healthy people(P>0.05);there was no significant difference among rs187084,rs352139,rs352140(P>0.05);there were differences in genotype and all=ele of rs743836 polymorphism(P<0.05).Multivariate logistic analysis of independent risk factors affecting sepsis showed that rs743836 polymorphism were independent risk factors of sepsis(P<0.05).Conclusion Patients with trauma and pancreatitis with TLR9 rs743836 gene polymorphism in might be proned to sepsis risk.
