摘 要:目的了解灵山县土源性线虫流行情况及其影响因素,评价防治效果,为制定今后防治策略与措施提供科学依据。方法2016-2019年在灵山县按照地理方位将全县划分为东、西、南、北、中5个片区,在每个片区随机抽取1个乡镇的1个行政村为纵向监测点,连续4年开展土源性线虫病监测,监测对象为≥3岁常住居民,每个点约200人,共约1000人,收集监测对象粪样,用改良加藤厚涂片法一送二检,检测寄生虫虫卵。结果2016-2019年灵山县共调查4363人,土源性线虫总感染率为1.08%(47/4363),其中钩虫、蛔虫和鞭虫的感染率分别为0.66%(29/4363)、0.30%(13/4363)、0.11%(5/4363)。不同年度感染率差异有统计学意义(X^(2)=10.387,P<0.05),总体呈下降趋势(X^(2)=8.045,P<0.05)。烟墩镇感染率最高为2.75%(26/945),人群感染率最高的是幼托儿童2.55%(11/431)。各虫种均以轻度感染为主,占总感染数89.36%(42/47)。钩虫感染主要集中于70岁及以上年龄组,鞭虫感染率较高为60~69岁年龄组,蛔虫则在0~9岁年龄组感染较高。结论灵山县土源性线虫感染率已降至较低水平,有地区和人群的分布差异,提示要针对不同地区、不同人群制定精准的防控措施。Objective To determine the epidemic trend of soil-transmitted helminths infections and the prevalent risk factors in Lingshan county,Guangxi,and provide scientific basis for the evaluation and improvement of existing control measures and strategies.Methods One administrative village was selected randomly as surveillance site from the east,south,west,north,and center of Lingshan county.About 200 inhabitants aged over 3 each year during the period of 2016-2019 were selected for survey using random cluster sampling.Stool samples were collected and examined microscopically for the eggs of soil-transmitted helminths using the modified Kato-Katz thick smear method.Infection rates were compared using chi-square test.Results In total,4363 individuals were surveyed.The total infection rate of soil-transmitted helminths was 1.08%(47/4363).The infection rates of hookworm,roundworm and whipworm were 0.66%(29/4363),0.30%(13/4363)and 0.11%(5/4363),respectively.The infection rates tend to decrease during 2016-2019.The infection rate(2.75%,26/945)in Yandun town was the highest,compared to those of other towns,and that of preschool children(2.55%,11/431)was the highest,compared to those of other occupations.The infection intensity of various species of soiltransmitted helminths was relatively light.The proportion of light infection was 89.36%.The infection rate of hookworm was higher in the age group of≥70 than the other age groups;the infection rate of whipworm was higher in the age group of 60-69 while that of roundworm was higher in the age group of 0-9 than the other age groups.Conclusions The overall infection rate of soil-transmitted helminths in Lingshan county had been reduced to a relatively low level.However,the infection rate of soil-transmitted helminths varies among towns and population group.Different control measures and strategies should be considered.
