摘 要:目的分析血清1,25-二羟基维生素D3[1,25-(OH)_(2)VitD3]表达水平及维生素D受体(VDR)基因rs2228570 T/C位点单核苷酸多态性(SNP)分布特征与肺结核(TB)的易感性。方法收集2020年3月-2021年5月中国科学院大学深圳医院(光明)确诊的肺结核患者172例为TB组,同期健康人群138人为对照组。分别检测两组血清中1,25-(OH)_(2)VitD3水平,同时对VDR基因rs2228570 T/C位点SNP进行分析。结果TB组患者血清1,25-(OH)_(2)VitD3水平为(12.06±3.92)nmol/L,明显低于对照组(41.37±5.14)nmol/L,差异有统计学意义(t=6.064,P<0.05)。TB组VDR基因rs2228570 T/C位点CC基因型和C等位基因频率分别为47.67%和66.57%,明显高于对照组11.59%和31.16%,差异有统计学意义(χ^(2)=6.902、4.527,P均<0.05),且CC基因型和C等位基因患TB的相对风险增加(OR=1.963,95%CI:1.205~3.013和OR=1.605,95%CI:1.302~2.873)。VDR基因rs2228570 T/C位点CC基因型TB患者血清1,25-(OH)_(2)VitD3水平为(7.25±2.61)noml/L,明显低于TC和CC基因型(15.83±4.29)nmol/L和(17.64±4.80)nmol/L,差异有统计学意义(t=3.975、4.135,P<0.05),但TC和CC基因型TB患者1,25-(OH)_(2)VitD3水平之间差异无统计学意义(t=1.133,P>0.05)。结论TB患者血清1,25-(OH)_(2)VitD3水平明显降低,可能与TB的发病有密切关系。同时VDR基因rs2228570 T/C位点呈多态性,且CC基因型TB患者1,25-(OH)_(2)VitD3水平更低,可能是深圳地区TB发病的易感基因之一。Objective To analyze the level of 1,25-dihydroxyvitaminD3[1,25-(OH)_(2)VitD3]and Vitamin D receptor(VDR)gene rs2228570 T/C locus single nucleotide polymorphism(SNP)and susceptibility to tuberculosis(TB).Methods 172 tuberculosis patients diagnosed in Shenzhen Hospital(Guangming)of University of Chinese Academy of Sciences from March 2020 to May 2021 were enrolled as TB group,and 138 healthy people were enrolled as control group.The level of 1,25-(OH)_(2)VitD3 in the two groups were detected,and SNP in RS2228570 T/C locus of VDR gene was analyzed.Results The level of 1,25-(OH)_(2)VitD3 in TB group was(12.06±3.92)nmol/L,significantly lower than(41.37±5.14)nmol/L in control group(t=6.064,P<0.05).The CC genotype and C allele frequencies of RS2228570 T/C locus of VDR gene in TB group were 47.67%and 66.57%,respectively,which were significantly higher than those in control group(11.59%and 31.16%)(χ^(2)=6.902,4.527,P<0.05).The relative risk of TB was increased for CC genotype and C allele(OR=1.963,95%CI:1.205-3.013 and OR=1.605,95%CI:1.302-2.873).The level of 1,25-(OH)_(2)VitD3 in TB patients with CC genotype at RS2228570 T/C locus of VDR gene was(7.25±2.61)nmol/L,significantly lower than(15.83±4.29)nmol/L and(17.64±4.80)nmol/L of TC and CC genotype(t=3.975,4.135,P<0.05),but there was no significant difference in the level of 1,25-(OH)2 VitD3 between TC and CC genotype TB patients(t=1.133,P>0.05).Conclusions The level of1,25-(OH)2 VitD3 in TB patients decreased significantly,which might be closely related to the incidence of TB.At the same time,the RS2228570 T/C locus of VDR gene was polymorphic,and the level of 1,25-(OH)_(2)VitD3 in TB patients with CC genotype was lower,which might be one of the susceptibility genes of TB in Shenzhen.
