摘 要:目的 比较高海拔地区硬通道穿刺引流与非手术治疗基底节区中小量高血压性脑出血的疗效。方法 选取2015-01至2020-12武警青海总队医院收治的高血压性脑出血患者80例,出血部位均为基底节区,出血量20~40 ml,按随机数字表法分为两组:硬通道穿刺引流术为观察组,非手术治疗为对照组,分析比较两组治疗后语言障碍、肢体功能障碍等后遗症,比较美国国立卫生卒中量表(NIHSS)和日常生活能力评定(Barthel Index, BI)评分。结果 治疗3周后,观察组与对照组语言障碍发生率分别为2.5%和25.0%,肢体功能障碍发生率分别为5.0%和27.5%,两组比较差异有统计学意义(均P<0.01)。治疗3周后,观察组NIHSS评分(12.08±4.28)明显低于对照组(16.50±6.60),BI评分(29.33±8.46)高于对照组(20.48±6.47),差异有统计学意义(均P<0.01)。结论 高海拔地区应用硬通道穿刺引流治疗基底节区中小量高血压性脑出血的临床疗效优于非手术治疗。Objective To explore the clinical efficacy of hard channel puncture and drainage and conservative treatment of small and medium hypertensive cerebral hemorrhage in basal ganglia on high altitude areas.Methods Eighty patients of hypertensive cerebral hemorrhage(20-40 ml) in basal ganglia admitted to our hospital from January 2015 to December 2020 were randomly divided into two groups: the observation group(hard channel puncture drainage) and the control group(internal conservative treatment). The sequelae of language impairment, limb dysfunction, NIHSS score, and Barthel Index(BI) score were analyzed and compared respectively.Results After 3 weeks of therapy, the incidence of language impairment of the observation and control group were 2.5% and 25.0%, the incidence of limb dysfunction of both groups were 5.0% and 27.5%, respectively, and the differences were statistically significant(P<0.01). The NIHSS of the observation group(12.08±4.28) was significantly lower than that of the control group(16.50±6.60) after 3 weeks therapy(P<0.01), and the Barthel Index of the observation group(29.33±8.46) was better than that of the control group(20.48±6.47, P<0.01).Conclusions The treatment of hard channel puncture and drainage is better than the conservative treatment of hypertensive cerebral hemorrhage in basal ganglia on high altitude areas.
