摘　　要：目的通过系统性文献检索和Meta分析,明确莫达非尼对改善健康成年人认知功能的有效性。方法通过计算机检索Pubmed、Web of Science和Cochrane Library数据库,纳入莫达非尼用于改善健康成年人认知功能的临床随机对照实验,采用RevMan5.4软件进行数据合并分析。结果共纳入14项文献进行研究,其中7项交叉试验(248例受试者),7项平行对照试验(328例受试者,莫达非尼组143例,安慰剂组185例)。采用逆方差法Meta分析结果显示,与安慰剂组相比,莫达非尼改善健康成年人认知功能的整体效应具有统计学差异(SMD=0.11,95%CI=0.02-0.21,Z=2.46,P=0.01,I^(2)=70%)。亚组分析显示,莫达非尼对认知功能各个模块的改善均无统计学差异。结论莫达非尼对健康成年人的认知功能具有一定的改善作用。Objective To determine the efficacy of modafinil for cognitive enhancement in healthy adults by means of literature retrieval and meta-analysis.Methods Such databases as Pubmed,Web of Science and Cochrane Library were extensively searched.Eligible randomized clinical trials that evaluated the efficacy of modafinil versus placebo for cognitive function in healthy adults were included and analyzed by using RevMan5.4.Results A total of 14 trials involving 7 crossover studies(n=248)and 7 parallel studies(n=328,143 of the cases received modafinil and 185 received placebo)were included in the final analysis.Generic inverse variance meta-analysis indicated that there was statistically significant difference in the overall effect between modafinil and placebo(SMD=0.11,95%CI=0.02-0.21,Z=2.46,P=0.01,I^(2)=70%).Subgroup analysis did not indicate significant difference in cognitive function tests between different groups.Conclusions Modafinil can help improve cognition to some extent.Because of the limited evidence available,further studies are needed to elucidate the applicability of modafinil as a“smart”drug.
