自拟益气排膜汤促进正常分娩后子宫复旧临床研究

摘　　要：目的研究自拟益气排膜汤对正常分娩后子宫复旧的影响。方法选择2018年9月—2019年3月顺产且首次分娩的产妇,按照入组先后顺序将产妇随机分为实验组、阳性药物对照组和空白对照组,每组各80例。实验组产后予以益气排膜汤口服,阳性药物对照组产后予以新生化颗粒口服,空白对照组产后不予药物处理,疗程均为2周。检测3组产妇的药物安全性、子宫宫底高度、阴道血性恶露时间及阴道恶露持续时间、血液流变学相关指标。结果自拟益气排膜汤安全性与新生化颗粒相当(P>0.05);新生化颗粒可轻微加快产后子宫底下降(P<0.05),显著缩短阴道恶露持续时间、减少晚期产后出血及感染(P<0.05)的发生,及轻微改善产后血液流变学(P<0.05);益气排膜汤在临床疗效上显著优于新生化颗粒,除具有与新生化颗粒相当的作用外,同时还具有显著缩短血性恶露持续时间(P<0.05),及进一步收缩子宫(加快宫底下降及缩短耻上未及时间)和改善血液流变学的作用(P<0.05)。结论自拟益气排膜汤安全性良好,且能明显促进正常分娩后的子宫复旧,效果显著优于新生化颗粒。Objective To investigate the effect of Self-made Yiqi Paimei Decoction for uterine involution after normal delivery.Methods From September 2018 to March 2019,maternal with normal and first delivery in our hospital were randomly divided into experimental group,positive drug control group and blank control group,80 cases in each group.Experimental group was treated with Yiqi Paimo Decoction,the positive drug control group was treated with Xinshenghua Granule,and the blank control group was not treated with drugs after delivery.The course of treatment was 2 weeks.The drug safety,uterine fundus height,duration of vaginal haemorrhagic lochia,and hemorheology indexes of the three groups were detected.Results The safety of Self-made Yiqi Paimo Decoction was similar to that of Xinshenghua Granule(P>0.05);Xinshenghua Granule could slightly accelerate the decline of uterine fundus(P<0.05),significantly shorten the duration of vaginal lochia,reduce the secondary postpartum hemorrhage and infection(P<0.05),and slightly improve postpartum blood.Yiqi Paimo Decoction was superior to Xinshenghua Granule in clinical efficacy.It not only had the same effect as Xinshenghua Granule,but also had the effect of shortening the time of vaginal blood lochia(P<0.05).And it was further contracted the uterus(accelerated the decline of uterine fundus and shortened the time beyond shame)and improved hemorheology(P<0.05).Conclusion Self-made Yiqi Paimo Decoction has good safety and can obviously promote uterine involution after normal childbirth.The effect is significantly better than that of Xinshenghua Granule.
