中药外敷配合切开复位钢板螺钉固定治疗桡骨远端骨折临床观察

摘　　要：目的探讨中药外敷配合切开复位钢板螺钉固定治疗桡骨远端骨折的临床效果。方法选择2016年10月—2018年11月接受治疗的桡骨远端骨折患者86例,按随机数表法分为两组,每组43例。两组均行切开复位钢板螺钉固定治疗,对照组术后予以常规西药治疗和运动指导,在此基础上予以观察组中药外敷治疗。进行为期1年的随访,比较两组临床疗效、患肢关节功能及并发症发生情况。结果观察组优良率高于对照组,差异有统计学意义(P<0.05);观察组治疗3个月后患肢关节尺偏、桡偏、背伸、旋前、旋后及掌曲角度均大于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率(2.33%)低于对照组(16.28%),差异有统计学意义(P<0.05)。结论在切开复位钢板螺钉固定治疗基础上予以桡骨远端骨折患者中药外敷可取得确切治疗效果,能够有效改善患者术后的患肢关节功能,有助于降低并发症风险,具有推广价值。Objective To investigate the clinical effect of external application of traditional Chinese medicinal herbs combined with open reduction and plate screw fixation in the treatment of distal radius fracture.Methods 86 patients with distal radius fracture who were treated in our hospital from October 2016 to November 2018 were selected and divided into two groups according to the random number table(43 cases in each group).Both groups were treated with open reduction and plate and screw fixation,and the control group was treated with conventional Western medicine and exercise guidance after surgery.On this basis,the observation group was treated with external application of traditional Chinese medicine.A one-year follow-up was conducted to compare the clinical efficacy,joint function and complications of the two groups.Results The excellent and good rate in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).After 3 months of treatment,the ulnar deviation,radial deviation,dorsal extension,anterior rotation,posterior rotation and palmar flexion Angle of the affected limb in the observation group were all higher than that in the control group,with statistically significant differences(P<0.05).The complication rate of the observation group(2.33%)was lower than that of the control group(16.28%),and the difference was statistically significant(P<0.05).Conclusion On the basis of open reduction and plate and screw fixation,external application of traditional Chinese medicine in patients with distal radius fracture can achieve a definite therapeutic effect,effectively improve the postoperative joint function of the affected limb,help to reduce the risk of complications,and have a value of promotion.
