自拟逍遥滋肾饮结合周期疗法对围绝经期综合征临床疗效观察

摘　　要：目的:探讨自拟逍遥滋肾饮结合中药周期疗法治疗肝郁肾虚围绝经期综合征的临床疗效。方法:收集2016年4月—2018年3月于广州医科大学附属第三医院中医门诊就诊的60例肝郁肾虚型的围绝经期综合征患者随机分为两组,治疗组30例予自拟逍遥滋肾饮治疗,服药2个月;对照30例予常规克龄蒙片治疗,连用21 d。观察治疗1个月、2个月后KI(Kupperman Index)值,2个月后促卵泡雌激素(FSH)、促黄体生成素(LH)、雌二醇(E2)变化。结果:(1)1个月和2个月后,两组KI值均显著低于治疗前(P<0.01);组间比较,1个月后KI下降的幅度无差异(P>0.05),但2个月后治疗组KI下降的幅度比对照大(P<0.05)。(2)两组经治疗后FSH、LH均较治疗前明显降低(P<0.01),两组间比较均无差异(P>0.05);两组E2均较治疗前明显升高(P<0.01),两组间比较无差异(P>0.05)。(3)治疗组总有效率为93.3%,对照组为73.3%。两组之间存在差异(P<0.05)。(4)治疗组在治疗的2个月中,未出现不良反应;对照组在治疗中2例出现恶心纳差,3例出现乳房胀痛症状。结论:自拟的逍遥滋肾饮结合中药周期疗法对肝郁肾虚型围绝经期具有较好的临床疗效,能够显著改善患者症状以及体征,并能改善卵巢内分泌功能,使血中E2水平明显增高,FSH、LH水平下降,并且无一例不良反应发生,具有较好的安全性,值得临床推广及使用。Objective:To observe the clinical effect of self-made Xiaoyao Zishen Yin(main drugs:Bupleurum chinense,Poria cocos,Paeonia lactiflora,Salvia miltiorrhiza,Licorice,Gardenia jasminoides,Angelica sinensis,Cuscuta seed,Ligustrum lucidum)combined with traditional Chinese medicine periodic therapy on perimenopausal syndrome of liver depression and kidney deficiency.Methods:From April 2016 to March 2018,60 cases of perimenopausal syndrome with kidney deficiency and liver depression were randomly divided into two groups.The treatment group was treated with Xiaoyao Zishen Yin for 2 months,while the control group was treated with routine KeLingMeng Tablets for 21 days.The changes of KI(Kupperman Index)and follicle stimulating hormone(FSH),luteinizing hormone(LH)and estradiol(E2)were observed after 1 and 2 months of treatment.Results:(1)After 1 and 2 months treatment,the KI values of the two groups were significantly lower than those before treatment(P<0.01);there was no difference between the two groups after one months(P>0.05),but after two months,the KI values of the treatment group were significantly lower than those of the control group(P<0.05).(2)After treatment,FSH and LH in both groups were significantly lower than those before treatment(P<0.01),and there was no difference between the two groups(P>0.05);E2 in both groups was significantly higher than that before treatment(P<0.01),and there was no difference between the two groups(P>0.05).(3)The total effective rate was 93.3%in the treatment group and 73.3%in the control group.There was a difference between the two groups(P<0.05).(4)There were no adverse reactions in the treatment group for 2 months,while in the control group,there were 2 cases of nausea and 3 cases of breast swelling and pain.Conclusion:The self-made Xiaoyao Zishen Yin combined with traditional Chinese medicine cycle therapy has a good clinical effect on perimenopause of liver depression and kidney deficiency type,can significantly improve the symptoms and signs of patients,and can improve ovarian e
