养肺固肾汤治疗慢性阻塞性肺疾病的临床疗效及对肺功能和运动耐力的影响

摘　　要：目的探讨养肺固肾汤治疗慢性阻塞性肺疾病的临床疗效及对肺功能和运动耐力的影响。方法选择慢性阻塞性肺疾病稳定期患者(肺肾两虚证)115例,依据治疗方式的不同分为观察组(n=61例)和对照组(n=54例),对照组患者采用沙美特罗替卡松粉吸入治疗,观察组在对照组治疗的基础上,口服养肺固肾汤治疗。两组均治疗3个月。比较两组治疗疗效,比较两组治疗前后中医证候积分比较,比较两组治疗前后肺功能指标(第1秒用力呼气量(FEV1)、用力肺活量(FVC)、第1秒用力呼气容积占用力肺活量的百分比(FEV1/FVC))变化,比较两组治疗前后6 min步行距离(6MWD)、肺康复评定指数(BODE)评分、血清C反应蛋白(CRP)和降钙素原(PCT)水平变化。结果观察组治疗总有效率高于对照组(P<0.05);观察组治疗后中医证候积分低于对照组(P<0.05),观察组治疗后肺功能指标FEV1、FVC和FEV1/FVC改善幅度优于对照组(P<0.05),观察组治疗后6MWD和BODE评分改善幅度优于对照组(P<0.05);观察组治疗后血清CRP、PCT水平下降幅度优于对照组(P<0.05)。结论养肺固肾汤辅助治疗慢性阻塞性肺疾病疗效显著,可有效改善患者的临床症状和肺功能,降低炎症因子水平,提高运动耐力,值得临床推广应用。Objective To investigate the clinical efficacy of Yangfei Gushen Decoction(养肺固肾汤)in the treatment of chronic obstructive pulmonary disease(COPD)and its effect on lung function and exercise endurance.Methods A total of 115 patients with chronic obstructive pulmonary disease in stable period(syndrome of deficiency of lung and kidney)were selected and divided into observation group(61 cases)and control group(54 cases)according to different treatment methods.Patients in the control group were treated with salmeterol roticasone powder inhalation while those in the observation group were treated with Yangfei Gushen Decoction orally on the basis of treatment in the control group.Both groups were treated for 3 months.We compared two groups’curative effect,traditional Chinese medicine symptom integral,lung function index(forced expiratory volume 1 second(FEV1)and forced vital capacity(FVC),forced expiratory volume in 1 second,the percentage of forced vital capacity(FEV1/FVC))changes,6 min walking distance(6MWDS),pulmonary rehabilitation evaluation index(BODE)score,serum C-reactive protein(CRP)and calcitonin(PCT)level changes.Results The total effective rate in the observation group was higher than that in the control group(P<0.05).After treatment,the score of TCM symptoms in the observation group was lower than that in the control group(P<0.05).The improvement of pulmonary function indexes FEV1,FVC and FEV1/FVC in the observation group was better than that in the control group(P<0.05).The improvement of 6MWD and BODE scores in the observation group was better than that in the control group(P<0.05).After treatment,serum CRP and PCT levels in the observation group were significantly lower than those in the control group(P<0.05).Conclusion Yangfei Gushen Decoction has a significant curative effect in the adjuvant treatment of chronic obstructive pulmonary disease,which can effectively improve the clinical symptoms and lung function of patients,reduce the level of inflammatory factors,improve exercise endurance,an
