益肾通督合剂结合督脉灸对强直性脊柱炎患者疗效及腰椎功能、免疫功能的影响

摘　　要：目的探究益肾通督合剂结合督脉灸对强直性脊柱炎患者疗效、腰椎功能、免疫功能的影响。方法研究纳入79例强直性脊柱炎患者,均由医院2019年1月—2019年12月收治,采取随机数字表法将其分为两组,对照组患者(39例)常规西医疗法,观察组患者(40例)在对照组治疗基础上联合益肾通督合剂结合督脉灸治疗,比较两组患者临床疗效、治疗前后中医症状(颈腰臀背疼痛、怕风怕冷、四肢不温、疲倦乏力、晨僵等)积分变化、治疗前后患者巴氏病情活动指数(BASDAI)及巴氏功能活动指数(BASFI)变化、治疗前后患者血沉(ESR)、C反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子α(TNF-α)水平变化、治疗前后T细胞亚群指标变化、不良反应。结果观察组患者治疗总有效率(97.50%,39/40)高于对照组(82.05%,32/39),P<0.05;治疗前,两组患者颈腰臀背疼痛、怕风怕冷、四肢不温、疲倦乏力、晨僵中医症状积分、BASDAI指数评分及BASFI指数评分、ESR、CRP、IL-6、TNF-α、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)等T细胞亚群指标比较,P>0.05,治疗后各组患者颈腰臀背疼痛、怕风怕冷、四肢不温、疲倦乏力、晨僵中医症状积分、BASDAI指数评分及BASFI指数评分、ESR、CRP、IL-6、TNF-α、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)等T细胞亚群指标均改善,观察组患者治疗后颈腰臀背疼痛、怕风怕冷、四肢不温、疲倦乏力、晨僵中医症状积分、BASDAI指数评分及BASFI指数评分、ESR、CRP、IL-6、TNF-α、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)等T细胞亚群指标均优于对照组,P<0.05;观察组治疗不良反应率0.00%(0/40)低于对照组15.38%(6/39),P<0.05。结论益肾通督合剂结合督脉灸治疗强直性脊柱炎临床疗效显著,患者症状改善,腰椎功能恢复好,免疫功能提升,不良反应率低,安全可靠,值得应用。Objective To explore the effect of Yishen Tongdu Mixture(益肾通督合剂)combined with Du meridian moxibustion on the efficacy,lumbar function and immune function of patients with ankylosing spondylitis.Methods A total of 79 patients with ankylost spondylitis were included in this study,all of whom were admitted to our hospital from January 2019 to December 2019.They were divided into two groups by the random number table method.The control group(39 cases)received conventional western medicine therapy,while the observation group(40 cases)received combined treatment with Yishen Tongdu Mixture combined with Du meridian moxibustion on the basis of the control group.The clinical efficacy,integral changes of TCM symptoms(neck,waist,hip and back pain,fear of wind and cold,tepid limbs,fatigue,morning stiffness,etc.)before and after treatment,changes of pasteurized disease activity index(BASDAI)and pasteurized functional activity index(BASFI),ESR,C-reactive protein(CRP),interleukin-6(IL-6)and tumor necrosis factor α(TNF-α),changes of T cell subsets and adverse reactions before and after treatment were compared between the two groups.Results The total effective rate(97.50%,39/40)in the observation group was higher than that in the control group(82.05%,32/39)(P<0.05).Before treatment,the two groups′neck,waist,hip and back pain,fear of wind and cold,tepid limbs,fatigue,morning stiffness,BASDAI index score and BASFI index score,levels of ESR,CRP,IL-6,TNF-α,CD4^(+),CD8^(+)and CD4^(+)/CD8^(+)and other T cell subsets were not significant(P>0.05).After treatment,those in the observation group were better than those in the control group(P<0.05).The rate of adverse reaction(0.00%,0/40)in the observation group was lower than that in the control group(15.38%,6/39)(P<0.05).Conclusion Yishen Tongdu Mixture combined with Du meridian moxibustion for the treatment of ankylosing spondylitis has a significant clinical effect.The patients′symptoms improved,lumbar function recovered well and the immune function improved.The adverse
