养心通络汤联合麝香通心滴丸治疗冠心病心绞痛的临床研究

摘　　要：目的探讨养心通络汤联合麝香通心滴丸治疗气虚血瘀型冠心病心绞痛的疗效及对血清IL-18、sICAM-1和TNF-α水平影响。方法选取来该院接受治疗的82例气虚血瘀型冠心病心绞痛患者,随机分为两组各41例。对照组患者给予麝香通心滴丸,实验组在对照组基础上口服养心通络汤,两组患者均持续治疗4周。对比两组患者的临床疗效、心绞痛发病情况、中医证候积分、心功能指标水平及血清IL-18、sICAM-1和TNF-α水平。结果治疗后,实验组总有效率明显较高(P<0.05);两组患者心绞痛持续时间、发作次数及硝酸甘油服药用量显著降低(P<0.05);并且实验组降低较明显(P<0.05);两组患者心悸、胸痛胸闷、气短、神疲乏力等证候积分显著降低(P<0.05);并且实验组降低较明显(P<0.05);治疗后两组患者LVESD和LVEDD值显著降低,LVEE显著升高(P<0.05);并且实验组改善较明显(P<0.05);两组治疗后血清IL-18、sICAM-1和TNF-α水平明显降低(P<0.05);且实验组降低较明显(P<0.05)。结论采用养心通络汤联合麝香通心滴丸治疗气虚血瘀型冠心病心绞痛具有较好的治疗效果,能够改善血清IL-18、sICAM-1和TNF-α水平,安全性较高,值得在临床上推广应用。Objective To investigate the efficacy of Yangxin Tongluo Decoction(养心通络汤)combined with Shexiang Tongxin Dropping Pill(麝香通心滴丸)in the treatment of angina pectoris of coronary heart disease with Qi deficiency and blood stasis and the effect on serum levels of IL-18,sICAM-1 and TNF-α.Methods Eighty-two patients with angina pectoris of coronary heart disease with Qi deficiency and blood stasis were randomly divided into two groups,41 patients in each group.Patients in the control group were given Shexiang Tongxin Dropping Pill and those in the experimental group were given Yangxin Tongluo Decoction orally on the basis of the control group.Patients in both groups were treated continuously for 4 weeks.The clinical efficacy,angina pectoris incidence,TCM syndrome score,cardiac function index level and serum levels of IL-18,sICAM-1 and TNF-αwere compared between the two groups.Results After treatment,the total effective rate of the experimental group was significantly higher(P<0.05).The angina pectoris duration,attacks and nitroglycerin dosage were significantly reduced in both groups(P<0.05).In the experimental group,the decrease was more significant(P<0.05).After treatment,symptoms such as palpitation,chest pain,chest tightness,shortness of breath and mental fatigue were significantly reduced(P<0.05).In the experimental group,the decrease was more significant(P<0.05).After treatment,LVESD and LVEDD were significantly reduced and LVEE was significantly increased in both groups(P<0.05).The improvement of the experimental group was significant(P<0.05).After treatment,serum levels of IL-18,sICAM-1 and TNF-αin both groups were significantly decreased(P<0.05).The decrease in the experimental group was significant(P<0.05).Conclusion Yangxin Tongluo Decoction combined with Shexiang Tongxin Dropping Pill has a good therapeutic effect on angina pectoris of coronary heart disease with Qi deficiency and blood stasis,and can improve serum levels of IL-18,sICAM-1 and TNF-α.It is safe and worthy of clinical ap
