摘  要:目的观察电针对膜迷路积水模型豚鼠耳蜗形态及血浆环磷酸腺苷(cAMP)、水通道蛋白2(AQP2)浓度变化的影响,探讨电针对梅尼埃病膜迷路积水的干预作用及可能调节机制。方法健康雄性豚鼠24只随机分为空白组、模型组、电针组,每组8只。除空白组,其余2组均通过腹腔注射血管加压素建立膜迷路积水模型。空白组不予干预;模型组造模后采取与电针组相同固定方式;电针组取"百会""听宫"穴,采用2/15 Hz频率进行治疗,留针20 min,每天治疗1次,连续治疗10 d。干预结束后,检测耳蜗积水程度,并计算R值,检测血浆cAMP、AQP2浓度。结果(1)与空白组比较,模型组豚鼠耳蜗R值增加(P<0.01);与模型组相比,电针组豚鼠耳蜗R值降低(P<0.01)。(2)与空白组比较,模型组豚鼠血浆cAMP浓度增加(P<0.01);与模型组相比,电针组豚鼠血浆cAMP浓度降低(P<0.01)。(3)与空白组比较,模型组豚鼠血浆AQP2浓度增加(P<0.01);与模型组相比,电针组豚鼠血浆AQP2浓度降低(P<0.01)。结论电针"百会""听宫"能减轻豚鼠膜迷路积水,其作用机制可能与下调血浆cAMP、AQP2的表达有关。Objective To observe the effect of electro-acupuncture(EA)on the morphology of cochlea and levels of plasma cyclic adenosine monophosphate(p-cAMP)and aquaporin-2(p-AQP2)so as to explore the potential mechanism for the treatment of endolymphatic hydrops(EH)in Meniere′s disease by EA.Methods Twenty-four healthy male guinea pigs were randomly divided into blank group,model group and EA group,eight animals per group.Animals except blank group were injected with arginine vasopressin to establish EH.No treatment was given to the blank group.Guinea pigs in model group were fixed with the same method as the EA group.Guinea pigs in the EA group were treated with EA at"Baihui(DU20)"and"Tinggong(SI19)"with current frequencies of 2/15 Hz once daily for consecutive 10 days.After treatment,the severity of cochlear hydrops was evaluated,the ratio of scala media(SM)area to SM+scala vestibuli(SV)area(R value)was calculated.Concentrations of p-cAMP and p-AQP2 were observed.Results(1)R value in the model group increased compared with that of the blank group(P<0.01).Compared with that in the model group,R value in the EA group was significantly reduced(P<0.01).(2)Guinea pigs in the model group showed significantly increased p-cAMP concentrations compared with that in the blank group(P<0.01).In contrast,animals in the EA group showed a lower concentration of p-cAMP than that in model group,and this significance reached statistical significance(P<0.01).(3)Guinea pigs in the model group showed significant increase from the blank group in p-AQP2 concentrations(P<0.01).In contrast,animals in the EA group showed a lower concentration of p-AQP2 than that in the model group,and this significance reached statistical significance(P<0.01).Conclusion EA treatment could suppress the development of endolymphatic hydrops,and the mechanism is likely to be related with down-regulating the concentrations of p-cAMP and p-AQP2.
