摘 要:目的了解累及血液系统的药品不良反应(ADR)的人群特点、关联药物及上报人员的专业背景。方法收集2018-2020年上报至市辖区ADR监测站,累及器官系统为血液系统的ADR报告,提取并分析病例的年龄、性别、ADR名称、严重程度、关联药物、治疗转归及关联性评价结果,以及上报人员的职业及所属专业等。结果共收集到累及血液系统的ADR报告360份,涉及患者360例,男女比例为1.61:1,以年龄65~80岁多见(占44.44%);严重的ADR占76.39%;ADR转归上以好转或治愈为主(占90.83%);ADR上报人员以临床药师为主(占72.78%),且ADR关联性评价准确率明显高于临床医师(83.59%vs42.11%);关联药物主要为抗肿瘤药物(占67.22%)、神经系统用药(占11.39%)、抗感染药物(占10.56%)及抗栓药物(占5.28%)。结论累及血液系统的ADR严重程度以严重的ADR为主,关联药物以抗肿瘤药物、神经系统用药及抗感染药物为多见。临床药师在血液系统ADR监测中发挥重要作用。AIM To investigate the population characteristics,related drugs and the professional background of reporting personnel of adverse drug reaction(ADR)reports involving the blood system.METHODS The ADR reports involving the blood system were collected from ADR monitoring center of the municipal distict from 2018 to 2020.The data was extracted and analyzed,including age,gender,ADR name,severity,related drugs,treatment outcome and relevance evaluation,as well as the occupation and specialty of the reporting personnel.RESULTS A total of 360 ADR reports involving the blood system were collected,involving 360 patients.The male to female ratio was 1.61:1,most of them were aged between 65 and 80 years old(44.44%).Serious ADRs accounted for 76.39%,and the main outcomes were improved or cured(90.83%).Clinical pharmacists were the main reporting personnel of ADR invovling the blood system(72.78%),and the accuracy rate of ADR relevance evaluation from them was higher than that of physicians(83.59%vs 42.11%).The related drugs of ADR involving the blood system were mainly antitumor drugs(67.22%),central nervous system drugs(11.39%),antimicrobial drugs(10.56%)and antithrombotic drugs(5.28%).CONCLUSION The severity of ADR involving the blood system is mainly serious ADR,and most of the related drugs were antitumor drugs,central nervous system drugs and antimicrobial drugs.Clinical pharmacists play an important role in blood system ADR monitoring.
