品管圈在药房断药管理中的应用与实践

摘　　要：目的改善药房断药情况,降低药品断药频次。方法为实现降低药品断药频次,故成立品管圈小组。以每周差错数为指标进行现状把握,利用鱼骨图找出主要原因,运用库存管理技术拟定对策并实施。以有形成果(目标达标率)和无形成果(圈员的成长率)来评价品管圈活动实施效果。结果通过建立计划外补货制度、定期调整药房药品请领计划、完善安全库存的设置等,将断药频次从活动前的每周104.75次降低至活动后57.25次,目标达成率为73.93%;全体圈员在品管手法的运用、责任心、积极性、沟通与协调等方面成长率为8%~20%。结论经过品管圈活动后药房断药频次显著降低。通过开展该活动能够深入发掘和分析在医院内部药品供应中出现的问题,并运用科学有效的对策解决问题,完善了院内药品供应制度,从而保证患者用药。AIM To improve drug shortage management and reduce the frequency of drug shortage in pharmacy.METHODS In order to reduce the frequency of drug disconnection,a quality control circle(QCC)group was established.The number of errors per week as the index was taken to grasp the current situation,the fishbone diagram was used to find out the main causes,and the inventory management technology was used to formulate and implement the countermeasures.The implementation effect of QCC activities was evaluated by tangible results(target compliance rate)and intangible results(growth rate of circle members).RESULTS By establishing an unplanned replenishment system,regularly adjusting the drug requisition plan of the pharmacy and improving the setting of safety inventory,the frequency of drug shortage was reduced from 104.75 times per week to 57.25 times per week,and the target achievement rate was 73.93%.The growth rate of all the QCC members in the application,responsibility,enthusiasm,communication and coordination of quality management techniques in the QCC was 8%-20%.CONCLUSION After the activities of the QCC,the frequency of drug shortage of the pharmacy decreases significantly.By carrying out this activity,we can deeply explore and analyze the problems in the internal drug supply of the hospital,use scientific and effective countermeasures to solve the problems,and improve the drug supply system in the hospital,so as to ensure the patients’medication.
