摘　　要：目的探讨中性粒细胞计数/淋巴细胞计数比值(NLR)对高血压患者发生抑郁的预测价值。方法根据汉密顿抑郁量表(HAMD)评分,将219例高血压患者分为无抑郁组190例(HAMD评分<20分)和抑郁组29例(HAMD评分≥20分)。比较两组患者的WBC计数、红细胞分布宽度(RDW)、血小板平均体积(MPV)、NLR及淋巴细胞计数/单核细胞计数比值(LMR)。采用logistic回归分析评估高血压患者发生抑郁的危险因素,采用受试者工作特征(ROC)曲线分析NLR对高血压患者发生抑郁的预测价值。结果抑郁组患者NLR高于无抑郁组,LMR低于无抑郁组(P<0.05),而两组患者WBC计数、RDW、MPV比较差异均无统计学意义(P>0.05)。Logistic回归分析结果显示,NLR是高血压患者发生抑郁的独立危险因素(OR=1.526,P<0.05)。ROC曲线分析结果显示,NLR预测高血压患者发生抑郁的ROC曲线下面积为0.783(95%CI 0.707~0.859,P<0.001),最佳截断值为2.594,对应的敏感度和特异度分别为82.80%、60.50%。结论NLR是高血压患者发生抑郁的独立危险因素,具有良好的预测价值。Objective To explore the predictive value of neutrophil count to lymphocyte count ratio(NLR)on depression in patients with hypertension.Methods According to Hamilton Depression Scale(HAMD)score,219 patients with hypertension were divided into non-depression group(190 cases,HAMD score<20 points)and depression group(29 cases,HAMD score≥20 points).WBC count,red blood cell distribution width(RDW),mean platelet volume(MPV),NLR and lymphocyte count to monocyte count ratio(LMR)between two groups were compared.Logistic regression analysis was used to evaluate the risk factors for depression in patients with hypertension.Receiver operating characteristic(ROC)curve was used to analyse the predictive value of NLR for depression in patients with hypertension.Results NLR in patients of depression group was higher than that in non-depression group,and LMR was lower than that in non-depression group(P<0.05),while there were no significant difference of WBC count,RDW and MPV between the two groups(P>0.05).Logistic regression analysis showed that NLR was an independent risk factor for depression in patients with hypertension(OR=1.526,P<0.05).ROC curve analysis showed that area under ROC of NLR for predicting depression in patients with hypertension was 0.783(95%CI 0.707~0.859,P<0.001),and the optimal cut off value was 2.954,the sensitivity and specificity were 82.80%and 60.50%respectively.Conclusion NLR is an independent risk factor for depression in patients with hypertension and it has good predictive value.
