摘　　要：目的探讨血管外皮瘤(HPC)和血管瘤型脑膜瘤(AM)MRI表现的差异,提高HPC的诊断水平。方法回顾性分析经手术和病理证实的21例HPC患者和23例AM患者的临床和MRI常规序列及DWI征象,比较两组肿瘤各征象的差异。结果HPC和AM组患者性别、肿瘤形态、信号均质性、增强扫描均质性、瘤内囊变坏死情况差异均有统计学意义(P值均<0.05),另外HPC组平均ADC值显著低于AM组,以ADC值<1.05×10-3mm2/s作为诊断HPC的标准,AUC为0.701,诊断效能中等,敏感性为66.7%,特异性为73.9%,准确性为70%。结论HPC和AM的MRI表现有一定的差异,肿瘤的ADC值可以作为常规MRI征象的必要补充有助于两者的鉴别诊断。Objective To investigate the difference of clinical and MRI features between hemangiopericytoma and angiomatous meningioma.Methods Clinical and MRI findings including DWI features of 21 patients with HPC and 23 patients with AM confirmed by pathology were retrospectively analyzed,and the differences of tumor signs between the two groups were compared.Results The gender of patients,shape of tumor,homogeneous signal,homogeneous enhancement,cystic degeneration or necrosis in tumor had significant difference between HPC and AM group(P<0.05).The ADC values of HPC group were significantly lower than that of the AM group,when the ADC value was lower than 1.05×10-3mm2/s the lesion which was diagnosed as HPC.The sensitivity was 66.7%,specificity of 73.9%,the accuracy is 70%.Conclusion There are several differences between HPC and AM.The combination of ADC value with the clinical and MRI features could be useful in diagnosis and differential diagnosis of the tumor.
