胸腔镜术后两种引流管固定方法对肋间神经刺激的研究

摘　　要：目的:探究胸腔镜术后患者留置胸腔闭式引流管与体表的不同角度对肋间神经的刺激程度,从而推断对患者产生疼痛的影响程度。方法:选取某三级甲等医院胸外科行胸腔镜手术治疗的80名患者,按照随机数字表法分为对照组和观察组,各40例,对照组采用传统方法对留置胸管进行固定,观察组在对照组固定方法基础上注意调整胸管与患者胸壁的角度,保持约90°。结果:对照组和观察组在性别、年龄、疾病种类、拔管时间等方面无统计学意义,在术后24、48、72 h疼痛评分有显著差异。结论:通过改变留置胸腔闭式引流管与患者胸壁的角度,可以减轻其对肋间神经的刺激程度,从而减轻患者的疼痛。Objective:To explore the degree of stimulation of intercostal nerve in patients with thoracic closed drainage tube indurated at different angles from the body surface after thoracoscopic surgery,so as to infer the degree of influence on patients’pain.Methods:Select a city in henan province,a certain level of first-class hospital thoracic line thoracoscope surgery treatment of 80 patients,using a convenience sampling method according to random number table divided into control group and observation group and control group,40 cases in the control group using traditional methods of chest tube fixed,fixed method of observation group in the control group of attention to adjust the chest tube with the patient’s chest wall Angle,Angle of about 90.Results:There was no statistical significance between the control group and the observation group in terms of gender,age,disease types,extubation time and other aspects,and there was significant difference in the pain score of 24 h,48 h and 72 h after surgery.Conclusion:By changing the angle between the indignant thoracic closed drainage tube and the chest wall of patients,the stimulation of the intercostal nerve can be alleviated,and the pain of patients can be alleviated.
