摘  要:目的:探讨急性ST段抬高型心肌梗死后微循环的病理改变、探索无复流的发生机制。方法:通过构建大鼠心肌梗死模型,对大鼠心肌梗死后不同时间段的心肌微循环病理改变进行观察。结果:闭塞大鼠冠状动脉血管1 h后,毛细血管内皮细胞轻度肿胀,管腔轻度狭窄;3~6 h后,毛细血管内皮细胞肿胀加重,管腔面积明显狭窄,管腔内红细胞淤滞。这种改变可能是大鼠急性心肌梗死模型微循环障碍的主要病理基础。结论:梗死中心区域的微循环改善的最佳时间窗应在大鼠冠状动脉闭塞后1 h左右,且越早越好,超过3 h再灌注,短期内微循环改善较为困难。Objective:To investigate the pathological changes of microcirculation after acute ST-segment elevation myocardial infarction and to explore the mechanism of non-reflux.Methods:The pathological changes of myocardial microcirculation in different periods after myocardial infarction were observed by constructing myocardial infarction model in rats.Results:1 h after occlusion of rat coronary arteries,the capillary endothelial cells were slightly swollen and the lumen was slightly narrow;3~6 h later,the capillary endothelial cells were significantly swollen,the lumen area was significantly narrow,and the lumen red blood cells were congested.Such changes may be the main pathological basis of microcirculation disorders in the rat model of acute myocardial infarction.Conclusion:The optimal time window for microcirculation improvement in the infarct center area should be about 1 h after coronary artery occlusion in rats,and the earlier the better.After 3 h of reperfusion,the short-term microcirculation improvement is relatively difficult.
