感冒清热颗粒致中毒性表皮坏死松解症

摘　　要：1例28岁男性患者因发热、头晕、头痛,口服感冒清热颗粒治疗(12g,2次/d)。3d后,患者腹部、四肢出现密集粟粒大小红疹,伴瘙痒。停用感冒清热颗粒并对症治疗。2 d后皮疹迅速泛发全身,周身水肿性红斑融合成片,其上可见散布水疱,背部、腰部皮肤有糜烂,双眼球结膜充血,口腔黏膜多发破溃,伴有高热。入院治疗第4d,患者躯干、四肢皮肤出现密集水疱、大疱,周身水疱面积>30%,疼痛明显。诊断为感冒清热颗粒致中毒性表皮坏死松解症。予以甲泼尼龙、人免疫球蛋白、维D钙咀嚼片、骨化三醇、奥美拉唑镁肠溶片、膦甲酸钠等治疗。15d后,患者皮疹治愈。A 28-year-old man with fever,dizziness,headache took Ganmaoqingre Granules 12 g twice daily.Three days later,he had dense miliary red rashes on his abdomen and extremities,accompanied by skin pruritus.Ganmaoqingre Granules was stopped and symptomatic treatment was given.Two days later,the patient developed red rashes all over the body rapidly.The edematous erythematous skin linked into pieces.The vesicles were visible on the surface.Meanwhile,skin and mucosal erosion appeared on his mouth,back and waist,accompanied by high fever and conjunctival congestion.On day 4 of treatment,Dense vesicles and bullae occurred on his trunk and extremities,accompanied by pain,BSA>30%.Ganmaoqingre Granules-induced toxic epidermal necrolysis was diagnosed.He was treated with methylprednisolone,human immunoglobulin,calcium supplement with vitamin D chewable tablets,calcitriol soft capsules,omeprazole Magnesium enteric-coated tablets,foscarnet sodium,et al.His rashes gradually subsided.Fifteen days later,the drug eruptions were cured clinically.
