复方芩柏颗粒坐浴联合九华膏治疗混合痔外剥内扎术后疗效观察

摘　　要：目的观察复方芩柏颗粒坐浴联合九华膏治疗混合痔外剥内扎术后疗效观察效果。方法将我院100例混合痔外剥内扎术后肛周疼痛症状患者随机分为观察组与对照组各50例。对照组采用高锰酸钾治疗,观察组采用复方芩柏颗粒坐浴联合九华膏治疗。比较两组疗效,出血、疼痛、水肿、评分,肛管最大收缩压、直肠静息压、高压区长度,血清血小板源生长因子(PDFG)、碱性成纤维细胞生长因子(b FGF)、白细胞介素-6(IL-6)、基质金属蛋白酶抑制因子-1(TIMP-1),疼痛消失时间、创面愈合时间。结果治疗后,观察组总有效率高于对照组(P<0.05);两组出血、疼痛、水肿评分、肛管最大收缩压、高压区长度、IL-6、TIMP-1水平均低于治疗前,观察组出血、疼痛、水肿评分、肛管最大收缩压、高压区长度、IL-6、TIMP-1水平均低于对照组(P<0.05);两组直肠静息压、PDFG、bFGF水平均高于治疗前;观察组直肠静息压、PDFG、bFGF水平高于对照组(P<0.05);观察组疼痛消失时间、创面愈合时间均小于对照组(P<0.05)。结论复方芩柏颗粒坐浴联合九华膏治疗混合痔外剥内扎术后肛周疼痛症状的效果显著,可影响胶原代谢指标的表达,促进胶原合成和沉积,提高肛门功能,加速创面愈合。Objective To observe the curatiue effect of Compound Qinbai Granules sitz bath combined with Jiuhua Ointment after mixed hemorrhoid exfoliation and internal ligation surgery.Methods One hundred patients with perianal pain symptoms after external hemorrhoid dissection and internal hemorrhoid ligation were randomly divided into observation group and control group,with 50 patients in each group.Control group was treated with potassium permanganate,and observation group was treated with Compound Qinbai Granules sitz bath combined with Jiuhua Ointment.The curative effects of the two groups were compared,including bleeding,pain,edema,score,anal maximum systolic pressure,rectal resting pressure,length of high-pressure area,serum platelet derived growth factor(PDFG),basic fibroblast growth factor(bFGF),interleukin-6(IL-6),matrix metalloproteinase inhibitor factor-1(TIMP-1),pain disappearance time,and wound healing time.Results After treatment,the total effective rate of observation group was higher than control group(P<0.05);the bleeding,pain,edema scores,maximum anal systolic pressure,length of high-pressure zone,IL-6,and TIMP-1 levels in both groups were lower than before treatment,and were lower in observation group than control group(P<0.05);the average rectal resting pressure,PDFG,and bFGF levels in both groups were higher than before treatment,and were higher in observation group than control group(P<0.05);the time for pain disappearance and wound healing in observation group were shorter than control group(P<0.05).Conclusion The combination of Compound Qinbai Granules sitz bath and Jiuhua Ointment has a significant effect on perianal pain symptoms after mixed hemorrhoid exfoliation and internal ligation surgery,which can affect the expression of collagen metabolism indicators,promote collagen synthesis and deposition,improve anal function,and accelerate wound healing.
