耳硬化症的中医证型和影像学分型的相关性研究

摘　　要：目的通过研究耳硬化症的中医证型和临床影像学分型,找出中医各证型和影像学分型的相关性;方法根据就诊时间的先后顺序,选取2017年1月-2018年10月在我科门诊和病房手术的耳硬化症患者共30例,每位患者给予颞骨高分辨率CT检查,同时进行辨证归组,通过Spearman相关性检验分析,找出其相关性;结果 30例患者中,气滞血瘀型占40%,脾胃虚弱型占30%,肝肾阴虚型占16.66%,肾阳虚型占13.33%,窗型和气滞血瘀型有明显的正相关(P <0.01),耳蜗型与肾阳虚型正相关,与肝肾阴虚型负相关(P <0.05),混合型与脾胃气虚型有正相关(P <0.01)与肝肾阴虚正相关(P <0.05);结论耳硬化症的中医证型和CT影像学分型之间有一定的相关型,这种相关性为我们中医证型的量化提供了客观的观察指标,也为我们中医治疗耳硬化症提供了临床指导。Objective To find out the correlative types of TCM syndromes and influential classification by studying TCM syndromes and clinical imaging classification of otosclerosis.Methods According to the order of visiting time,30 patients with otosclerosis who were operated on in our outpatient department and ward from January 2017 to October 2018 were selected.Each patient was given high resolution CT examination of temporal bone,at the same time,they were classified into groups according to syndrome differentiation,and were divided into groups by Spearma.Result Among the 30 patients,40%were qi stagnation and blood stasis type,30%were spleen and stomach weakness type,16.66%were liver and kidney yin deficiency type,13.33%were kidney yang deficiency type,window type and qi stagnation and blood stasis type were positively correlated(P<0.01),cochlear type was positively correlated with kidney yang deficiency type,negatively correlated with liver and kidney yin deficiency type(P<0.05),and mixed type was positively correlated with spleen and stomach qi deficiency type(P<0.05).Relevance(P<0.01)was positively correlated with liver and kidney yin deficiency(P<0.05).Conclusion There is a clear correlation between TCM syndrome types and CT imaging classification of otosclerosis,which provides objective observation indicators for the objectification of TCM syndrome types and clinical guidance for TCM treatment of otosclerosis.
