无保护会阴接生联合控制胎头娩出对产妇会阴裂伤及母婴预后的影响

摘　　要：目的探讨无保护会阴接生联合控制胎头娩出对产妇会阴裂伤及母婴预后的影响。方法选取2018年1月至2019年12月在威海市中心医院进行自然分娩的204例产妇为研究对象。根据随机数字表法分为对照组与观察组,每组102例。对照组自然分娩过程中实施会阴保护接生,观察组自然分娩过程中实施无保护会阴接生联合控制胎头娩出方法。比较两组产妇会阴裂伤情况、母婴预后以及产妇满意度。结果观察组会阴完整率高于对照组,会阴裂伤程度低于对照组,差异有统计学意义(Z=3.896,P=0.001<0.05)。两组母婴预后比较差异未见统计学意义(P>0.05)。观察组产妇满意度(97.06%,99/102)高于对照组(86.27%,88/102),差异有统计学意义(χ^(2)=7.765,P=0.005<0.05)。结论自然分娩过程中实施无保护会阴接生联合控制胎头娩出方法实用性较强,可减少产妇会阴裂伤,保持会阴完整,提高产妇满意度,且不会对母婴预后造成不良影响。Objective To investigate the effect of unprotected perineal delivery combined with controlled fetal head delivery on perineal laceration and maternal and infant prognosis.Methods A total of 204 parturients who gave birth naturally in Weihai Central Hospital from January 2018 to December 2019 were selected as the study subjects.According to the method of random number table,they were divided into control group and observation group,with 102 cases in each group.The control group was delivered with perineum protection during natural delivery,and the observation group was delivered with unprotected perineum delivery combined with head delivery control during natural delivery.The perineal laceration,maternal and infant prognosis and maternal satisfaction were compared between the two groups.Results The rate of perineum integrity in the observation group was higher than that in the control group,and the degree of perineum laceration was lower than that in the control group,with significant differences(Z=3.896,P=0.001<0.05).There was no significant difference between the two groups in maternal or infant prognosis(P>0.05).The maternal satisfaction degree in the observation group(97.06%,99/102)was higher than that in the control group(86.27%,88/102),and the difference was significant(χ^(2)=7.765,P=0.005<0.05).Conclusions The method of unprotected perineal delivery combined with control of fetal head delivery during natural delivery is more practical,which can reduce perineal laceration,maintain perineum integrity,improve maternal satisfaction,and will not cause adverse effects on the prognosis of mothers and infants.
