摘 要:目的:探究不同血流灌注指数(PI)下新生儿动脉血气与经皮血气检测指标的一致性,分析经皮血气检测指标对不同PI值下新生儿的评估价值。方法:选取新生儿重症监护室(NICU)收治的120例新生儿为研究对象,根据检测的PI值分为3组,PI≥1.4为A组,0.7≤PI<1.4为B组,PI<0.7为C组。监测3组TcPCO_(2)、TcPO_(2)、PaCO_(2)、PaO_(2)并进行相关性及一致性分析。结果:A组TcPCO_(2)、PaCO_(2)明显高于B、C组,TcPO_(2)、PaO_(2)明显低于B、C组(P<0.05);3组新生儿TcPCO_(2)与PaCO_(2)、TcPO_(2)与PaO_(2)均呈正相关;Bland-altman结果表明TcPCO_(2)与PaCO_(2)、TcPO_(2)与PaO_(2)均呈显著一致性。结论:不同PI值下新生儿中TcPCO_(2)与PaCO_(2)、TcPO_(2)与PaO_(2)的一致性显著,可以减少动脉血气分析次数,较真实反映新生儿氧合情况。Objective:To explore the consistency of neonatal arterial blood gas and percutaneous blood gas testing indexes under different blood perfusion index(PI)values,and to analyze the evaluation value of percutaneous blood gas test indexes for neonates under different PI values.Methods:A total of 120 neonates admitted to neonatal intensive care unit(NICU)were selected as research subjects,and were divided into three groups according to the detected PI value:group A with PI values≥1.4,group B with 0.7≤PI values<1.4,and group C with PI values<0.7.Transcutaneous partial pressure of carbon dioxide(TcPCO_(2)),transcutaneous oxygen pressure(TcPO_(2)),arterial partial pressure of carbon dioxide(PaCO_(2))and arterial partial pressure of oxygen(PaO_(2))of the three groups were monitored and their correlation and consistency were analyzed.Results:TcPCO_(2)and PaCO_(2)in group A were significantly higher than those in group B and C,while TcPO_(2)and PaO_(2)were significantly lower(P<0.05);TcPCO_(2)and PaCO_(2),PaCO_(2)and TcPO_(2)in the three groups were positively;the bland-altman results showed that TcPCO_(2),PaCO_(2),and TcPO_(2)were significantly consistent with PaO_(2).Conclusion:The consistency between TcPCO_(2),PaCO_(2)and TcPO_(2)and PaO_(2)is significant in neonates with different PI values,which can reduce the number of arterial blood gas analyses and reflect the neonatal oxygenation more truly.
