657例系统性红斑狼疮患者抗核抗体谱特点的分析

摘　　要：目的探究系统性红斑狼疮患者血清抗核抗体谱的特点,明确相关自身抗体在SLE中的诊疗意义.方法回顾性分析657例SLE患者血清相关自身抗体实验室检查结果特点.结果(1)男性和女性之间抗核抗体、抗dsDNA、抗Sm、抗核小体(NUC)、抗SSA、抗SSB、抗组蛋白(His)、抗U1-RNP抗体阳性率差异不具有统计学意义(P<0.05).(2)儿童组跟成人组比较上述抗体阳性率差异不具有统计学意义(P<0.05).(3)成人组较老年组抗dsDNA抗体、抗SSA抗体、抗U1-RNP抗体阳性率高,差异有统计学意义(P=0.01、P=0.031、P=0.017).(4)儿童组抗NUC抗体阳性率比老年组高,差异具有统计学意义.结论(1)男性和女性SLE患者自身抗体阳性率无统计学差异;儿童与成年人SLE患者自身抗体阳性率无统计学差异.(2)老年SLE患者特异性自身抗体阳性率较低,临床上应注意避免漏诊、误诊.(3)儿童SLE患者抗核小体抗体阳性率高,该抗体有望成为诊断儿童系统性红斑狼疮的一个特异性的自身抗体.Objective To investigate the Antinuclear antibidy spectrum of systemic lupus erythematosus,and to express the related antibodies in the dia-gnose of it.Methods Clinical and laboratory data of 657 casesofsystemic lupus erythematosuswere collected.Results(1)There was no significant difference in the positive rates of antinuclear antibody,anti dsDNA,anti SM,anti nucleosome(NUC),anti SSA,anti SSB,anti histone(his)and anti-u1-rnp antibodies between male and female(P<0.05).(2)There was no significant difference in the antibody positive rate between children and adults(P<0.05).(3)The positive rates of anti dsDNA antibody,anti SSA antibody and anti-u1-rnp antibody in the adult group were higher than those in the elderly group(P=0.01,P=0.031,P=0.017).(4)The positive rate of anti NUC antibody in children group was higher than that in the elderly group,and the difference was statistically significant.Conclusion(1)Thepositive rates of autoantibodies in SLE have no statistically significance in male and female.(2)The positive rate of autoantibodi-es is lower in the elderly,thus the elderly is likely to be substantially underdi-agnosed or misdiagnosed in clinical practice.(3)NUC may be a distinctive antidoby in juvenil SLE.
