后路椎弓根螺钉内固定治疗胸腰椎骨折的效果与缺点

摘　　要：目的 分析后路椎弓根螺钉内固定治疗胸腰椎骨折的效果与缺点.方法 选取我院自2010年5月至2019年5月收治的80例胸腰椎骨折患者,根据治疗方式的不同将患者分为观察组(40例)与对照组(40例).对照组患者行常规内固定术对胸腰椎骨折处进行治疗,观察组患者行后路椎弓根螺钉内固定对胸腰椎骨折处进行治疗.对比两组患者治疗前后的生理功能改善情况、治疗前后椎体前缘高度比以及矢状面凹角.结果 治疗前,两组患者神经功能差异较小(P>0.05),治疗后,观察组患者神经功能显著优于对照组患者(P<0.05);治疗前,两组患者椎体前缘高度比与矢状面凹角差异较小(P>0.05),治疗后,观察组患者椎体前缘高度比与矢状面凹角显著优于对照组患者(P<0.05);观察组出现断钉以及钉板松动等并发症例数为0,对照组患者出现断钉以及钉板松动等并发症例数为0,两组患者的术后并发症发生率差异较小(P>0.05).结论 采用后路椎弓根螺钉内固定对胸腰椎骨折患者进行治疗,可对患者的神经功能进行显著改善,并对患者脊椎正常生理弯曲进行恢复,加强患者脊柱的稳定性,值得广泛推广使用.Objective To analyze the effect and disadvantage of posterior pedicle screw fixation in the treatment of thoracolumbar fractures.Methods Eighty patients with thoracolumbar fractures admitted from May 2010 to May 2019 in our hospital were divided into observation group(40 cases)and control group(40 cases)according to different treatment methods.The patients in the control group were treated with conventional internal fixation for thoracolumbar fractures.The patients in the observation group were treated with posterior pedicle screw fixation.The improvement of spermatogenic function,the height ratio of anterior edge of vertebral body and sagittal concave angle before and after treatment were compared between the two groups.Results Before treatment,there was little difference in nerve function between the two groups(P>0.05).After treatment,the nerve function of the observation group was significantly better than that of the control group(P<0.05).Before treatment,there was little difference between the height ratio of the anterior edge of the vertebral body and the sagittal concave angle between the two groups(P>0.05).After treatment,the height ratio of the anterior edge of the vertebral body and the sagittal concave angle of the observation group were significantly better than that of the control group.The number of patients in the observation group was 0,the number of patients in the control group was 0.There was little difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion The treatment of thoracolumbar fracture with posterior pedicle screw fixation can significantly improve the neurological function of the patients,restore the normal physiological curvature of the spine,and enhance the stability of the spine,which is worth popularizing widely.
