动力髋螺钉联合拉力螺钉治疗62例老年髋部骨折的手术体会

摘　　要：目的 探讨动力髋螺钉联合拉力螺钉治疗老年髋部骨折患者的临床效果.方法 选取我院2014年1月至2019年1月收治的62例70~81岁的老年髋部骨折患者,采用动力髋螺钉联合拉力螺钉手术方式,统计患者手术时间、术中出血量,并进行3个月~3年随访,评价分析髋关节Harris功能和术后并发症情况.结果 本组手术时间(52.05±12.6)min,术中出血量(100.12±18.05)mL.骨折愈合59例,骨折愈合时间(5.52±0.59)个月.1例患者因术后早期下地负重骨折移位、内固定松动;2例患者术后15个月x线复查股骨头缺血坏死、骨性关节炎;无深静脉血栓形成发生.髋关节Harris功能评分:优42例,良14分,中3分,差3例,优良率90.32%(56/62).结论 采用动力髋螺钉联合拉力螺钉手术治疗老年髋部骨折,手术时间短,髋关节功能恢复快,并发症发生率低.Objective To explore the clinical effect of dynamic hip screw combined with lag screw in the treatment of hip fracture in the elderly.Methods 62 cases of hip fracture aged 70~81 years were selected from our hospital from January 2014 to January 2019,Dynamic hip screw combined with tension screw was used.The time of operation and the amount of bleeding were Counted,The patients were followed up for 3 months to 3 years,Evaluation of Harris function and postoperative complications of hip joint.Results The operation time was(52.05±12.6)min,the intraoperative bleeding volume was(100.12±18.05)mL.There were 59 cases of fracture healing,the time of fracture healing was(5.52±0.59)months.One patient suffered from displacement and loosening of internal fixation due to early postoperative weight bearing fracture.X-ray examination showed avascular necrosis of the femoral head and osteoarthritis in 2 patients 15 months after operation,and no complications such as deep vein thrombosis occurred.Harris function score of hip joint:excellent 42 cases,good 14 points,medium 3 points,poor 3 cases,excellent and good rate 90.32%(56/62).Conclusion Dynamic hip screw combined with tension screw operation was used to treat hip fracture in the elderly.The operation time was short,the function of hip joint recovered quickly,and the incidence of complications was low.
